บทคัดย่อ

ผู้ป่วยระยะท้าย อาจเป็นผู้ป่วยที่ติดเตียงเป็นระยะเวลานาน มีการพยากรณ์โรคไม่ดี หรือป่วยด้วยโรคเรื้อรังที่ไม่สามารถช่วยเหลือตนเองได้ มักได้รับการดูแลแบบประคับประคองจากญาติหรือผู้ดูแลอย่างใกล้ชิด เมื่อผู้ป่วยอยู่ในระยะช่วงสุดท้ายของชีวิต จะได้รับคำบอกเล่าจากผู้ดูแลว่า “ผู้ป่วยอาการดีขึ้น ตื่นมาพูดคุย ความจำฟื้นกลับลุกขึ้นมาเดิน ลุกนั่งบนเตียง รับประทานอาหาร ร้องเพลง ยิ้มหัวเราะ” นับเป็นสิ่งที่น่ายินดีสำหรับญาติและผู้ดูแล แม้ภาวะแสงสุดท้ายเกิดในผู้ป่วยจำนวนน้อย ไม่ได้เกิดขึ้นกับผู้ป่วยทุกราย แต่กระนั้นก็ตาม หากอาการเหล่านี้เกิดขึ้นในผู้ป่วยระยะท้ายและไม่สามารถคาดการณ์ได้ล่วงหน้า อาจหมายถึงภาวะแสงสุดท้ายซึ่งเกิดจากสภาวะเครียดของร่างกายหรือเนื้อเยื่อที่พร่องออกซิเจนจะกระตุ้นให้มีการหลั่งสารสื่อประสาทในสมองมากกว่าปกติหรือคลื่นไฟฟ้าในสมองมีการเปลี่ยนแปลง ส่งผลให้ผู้ป่วยตื่นตัวมากขึ้น ความจำฟื้นกลับมา การรับรู้เพิ่มขึ้น ซึ่งเป็นปรากฏการณ์ที่เกิดขึ้นในช่วงระยะเวลาสั้น หลังจากนั้นร่างกายจะแย่ลงและเสียชีวิตในที่สุด ส่งผลให้ญาติต้องเผชิญหน้ากับความเศร้าโศกและการสูญเสีย  อย่างไรก็ตาม ภาวะแสงสุดท้ายยังมีการกล่าวถึงในเชิงวิชาการไม่มากนัก และเพื่อให้ญาติผู้ป่วย ผู้ดูแล รวมถึงบุคลากรทางการแพทย์ได้เข้าใจข้อมูลทางวิชาการเกี่ยวกับภาวะแสงสุดท้าย สามารถประเมินอาการของภาวะแสงสุดท้าย รวมถึงการให้ญาติเตรียมความพร้อมต่อการสูญเสียผู้ป่วยได้อย่างมีประสิทธิภาพ ดังนั้น บทความฉบับนี้เป็นการทบทวนวรรณกรรมเชิงบรรยาย มีวัตถุประสงค์เพื่ออธิบายลักษณะของผู้ป่วยระยะท้าย อาการของภาวะแสงสุดท้าย กลไกทางพยาธิสรีรวิทยาของการเกิดภาวะแสงสุดท้าย การเผชิญหน้ากับความเศร้าโศกและการสูญเสียของญาติ เมื่อผู้ป่วยอยู่ในสภาวะแสงสุดท้าย และบทบาทพยาบาลในการดูแลผู้ป่วยที่มีภาวะแสงสุดท้ายและครอบครัวของผู้ป่วย
