บทคัดย่อ

ภูมิหลัง: การสั่งสารอาหารทางหลอดเลือดดำ (TPN) เป็นเรื่องยุ่งยากซับซ้อน มีโอกาสเกิดความคลาดเคลื่อนทางยาได้สถาบันสุขภาพเด็กแห่งชาติมหาราชินีได้พัฒนาโปรแกรมการสั่ง TPN สำหรับผู้ป่วยเด็ก เพื่อช่วยในการคำนวณ และมีคำเตือนในการกำหนดสูตรที่เหมาะสม วัตถุประสงค์: เพื่อประเมินต้นทุนประสิทธิผลของโปรแกรมสั่ง TPN สำหรับผู้ป่วยเด็กในสถาบันสุขภาพเด็กแห่งชาติมหาราชินี (QIPT Program) เทียบกับโปรแกรมการสั่งอาหารแบบเดิม ในมุมมองของผู้ให้บริการทางสุขภาพ วิธีการ: ศึกษาประสิทธิผลจากความคลาดเคลื่อนทางยาของคำสั่ง TPN ควบคู่กับศึกษาต้นทุนของการดำเนินการ TPN ในหอผู้ป่วยกุมารเวชกรรมในสถาบันสุขภาพเด็กแห่งชาติมหาราชินี ในช่วงปีงบประมาณปี พ.ศ. 2562 - พ.ศ. 2564 โดยประกอบด้วยต้นทุน 3 ส่วน ได้แก่ 1) ต้นทุนการผลิต TPN คิดโดยวิธีดั้งเดิม 2) ต้นทุนการสั่งและบริหาร TPN และ 3) ต้นทุนการพัฒนา QIPT Program คิดโดยต้นทุนรายกิจกรรม ผล: ต้นทุนการดำเนินการของการให้ TPN ด้วย QIPT Program เทียบกับโปรแกรมเดิม มีค่า 1,120.75 บาท และ 1,124.44 บาทต่อขวด ตามลำดับ ร้อยละของการลดลงของความคลาดเคลื่อนทางยาในกลุ่ม QIPT Program เมื่อเทียบกับโปรแกรมเดิมเท่ากับ 85.37 มีความเสี่ยงสัมพัทธ์ 0.146 ซึ่งเป็น protective effect เมื่อคำนวณอัตราส่วนต้นทุนประสิทธิผลส่วนเพิ่มต่อหน่วย (Incremental cost-effectiveness ratio; ICER) เท่ากับ –33.58 หมายถึง ต้นทุนลดลง 33.58 บาทต่อการป้องกันความคลาดเคลื่อนทางยา 1 ครั้ง การวิเคราะห์ความไวของตัวแปรพบว่า ต้นทุนค่าแรงการผลิตเป็นตัวแปรสำคัญทำให้เกิดความคุ้มค่า สรุป: เมื่อเปรียบเทียบ QIPT Program กับโปรแกรมเดิมพบว่าค่า ICER มีค่าเป็นลบ แสดงถึงความสามารถในการลดความคลาดเคลื่อนทางยาด้วยต้นทุนที่ต่ำกว่า
