ผลของการใช้ยา Dolutegravir ร่วมกับยาต้านไวรัสอื่น ต่อระดับ Serum Creatinine eGFR และความชุกของความผิดปกติของไตในผู้ป่วยติดเชื้อเอชไอวี โรงพยาบาลโพนทอง : การศึกษาย้อนหลัง
บทคัดย่อ
การวิจัยเชิงพรรณนาแบบย้อนหลังโดยใช้ข้อมูลผู้ติดเชื้อเอชไอวีจากฐานข้อมูลเวชระเบียนอิเล็กทรอนิกส์ โรงพยาบาลโพนทอง จังหวัดร้อยเอ็ด ระหว่างเดือนมกราคม พ.ศ. 2557 – ตุลาคม พ.ศ. 2566 ในกลุ่มประชากรที่รักษาด้วยยา Dolutegravir (DTG) ร่วมกับยาต้านไวรัสอื่นทั้งหมดจำนวน 256 คน นำมาวิเคราะห์เปรียบเทียบระดับ SCr และ eGFR ก่อนการรับยา หลังการรับยาสัปดาห์ที่ 4 และ 52 ใช้สถิติวิเคราะห์ความแปรปรวนทางเดียวแบบวัดซ้ำ (One-way repeated measures ANOVA) และหาความชุกของการเกิดความผิดปกติของไตจากค่าร้อยละ ผลการศึกษาพบว่า กลุ่มที่รักษาด้วยยา DTG ร่วมกับยาต้านไวรัสอื่นที่เข้าเกณฑ์การใช้สถิติวิเคราะห์จำนวน 85 คน ระดับ SCr มีค่าเฉลี่ยก่อนการรับยามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p<0.05) ในสัปดาห์ที่ 4 (0.16 ± 0.02) และ 52 (0.23 ± 0.18) ส่วนค่าเฉลี่ยของ eGFR ก่อนการรับยามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p<0.05) ในสัปดาห์ที่ 4 (-12.83± 1.70) และ 52 (-21.01 ± 1.15) แต่ในกลุ่มที่รักษาด้วยยา TDF + 3TC + EFV จำนวน 49 คน และ TDF + FTC + EFV จำนวน 48 คน ไม่พบการเปลี่ยนแปลงของระดับ SCr และ eGFR ความชุกของความผิดปกติของไตอยู่ในระยะไตวายเรื้อรังระดับ 3a สัปดาห์ที่ 52 จำนวน 18 คน (ร้อยละ 7.03) ระดับ 3b จำนวน 2 คน (ร้อยละ 0.78) สามารถสรุปได้ว่าการใช้ยา DTG ร่วมกับยาต้านไวรัสอื่นทำให้ระดับ eGFR เปลี่ยนแปลงอย่างมีนัยสำคัญตั้งแต่สัปดาห์ที่ 4 จึงควรติดตามการทำงานของไตอย่างต่อเนื่องในผู้ที่อยู่ในระยะไตวายเรื้อรังระดับ 3 ขึ้นไป และควรตรวจตัวบ่งชี้ทางชีวภาพสำหรับโรคไตที่แสดงถึงภาวะไตวายเรื้อรังเพิ่มเติม
