Effect of Medication System Development using the IPD Paperless system on Medication errors at Yasothon Hospital

Abstract
Yasothon Hospital, a 436-bed S-class general hospital, has implemented the IPD Paperless system across all units since April 2024. In the initial phase, problems such as delays and medication errors were observed due to unfamiliarity, miscommunication, and the previous medication system that mainly relied on paper-based communication between physicians and wards.
This quasi-experimental study aimed to evaluate the effects of implementing the IPD Paperless system on medication errors at Yasothon Hospital, comparing the incidence, rate, type, and severity level of medication errors before and after the implementation . The study population consisted of medication error incidents reported in the HRMS system of Yasothon Hospital, during two periods: 12 months before and after implementation (April 1, 2023 – March 30, 2024 vs April 1, 2024 – March 31, 2025). Statistical analysis included frequency, percentage, and incidence rates, with chi-square tests applied at a significance level of p < 0.05. Data collection was conducted from January 1 to June 15, 2025.
The results showed that after the implementation , the number of Transcribing errors, Dispensing errors, and Prescribing errors decreased by 49.50%, 29.04%, and 6.19%, respectively. However, Administration errors increased by 24.76% and Pre-dispensing errors increased by 43.37% occurred in the pharmacy preparation process. Comparing the rate per 1,000 patient-days, prescribing errors decreased from 8.11 to 6.90, transcribing errors from 2.60 to 1.19, and dispensing errors from 6.05 to 3.89, with statistically significant reductions in all categories (p < 0.001). Regarding severity, levels C–D decreased from 6,123 to 2,404 (46.56%), while those at levels E–F, declined from 15 to 5 cases. No errors were reported at severity levels G–I.
In conclusion, the implementation of the IPD Paperless system was effective in reducing medication errors in critical stages of the medication-use process.
