Research method: This was a qualitative research. by developing a storage system notification system Then collect the documents for the AEFI reporting from the hospital database. Then, in-depth interviews and group discussions were held for those in charge of immunization and those involved in the healthcare system. To acknowledge the problem and jointly find a way to develop the AEFI surveillance system. Quantitative data from the AEFI report database. Data were analyzed using descriptive statistics. The data obtained from the qualitative research were based on thematic analysis of content. Research results: The main problem found was the AEFI immunization notification system developed by improving the old guidelines. More as follows. 1) Added a screening form for vaccination recipients to provide complete information for AEFI symptom reporting. The screening form has been updated to include additional necessary information. Vaccine recipients can assess themselves when symptoms occur. 3) Add a notification system (Pop up) in the patient service system. so that the doctor, nurse or pharmacist can check the complete vaccination history Conclusion: An important problem of AEFI surveillance system is the notification or search of suspected AEFI patients. Therefore, the development of a post-immunity surveillance system is important to create a tool for the victim. The vaccine can assess the symptoms themselves. In the patient care system, there must be a system to notify providers of vaccination history for consideration of treating patients and forward information to the AEFI reporting system.
