รูปแบบการจ่ายยาแนวใหม่ที่บ้านผู้ป่วยโรคเรื้อรังในเขตเทศบาลตำบลบึงบูรพ์ อำเภอบึงบูรพ์ จังหวัดศรีสะเกษ ช่วงการระบาดโรคติดเชื้อ COVID-19
Abstract
This is a cross-sectional study. The objectives of this study were to develop the service system for the patients with chronic diseases which were diabetes mellitus, hypertension or chronic disease with none of complications for care who could get the dispensing at their own houses and also to discover the drug-related problems at the patients’ houses including create the primary care pharmacy system during the COVID-19 pandemic. The study area was Buengboon Municipality, Buengboon District, Si Sa Ket Province. The sample were 435 patients who got the service according to the defined criteria. The data was analyzed by using frequency and percentage. The information has been collected since January 2021 to February 2021.
The new dispensing model was keying the prescription, drug preparing, drug checking, fieldwork preparing, dispensing by the pharmacists, collecting the data of drug-related problems in the standard medication record format, medication reconciliation, drug decreasing, history taking and asking about symptoms, coordinating by the officers, driving guide, forwarding the data of drug-related problems to the physician. The results revealed that most of the 435 sample were female, 65.06 percent, and they had the average age of 68 years. The highest value of direct disbursement of medical expense was 40.22 percent. The highest 3 types of congenital disease were 45.06 percent. The leftover drug prices were 13,600 baht. The first three rank of leftover medicine lists were Simvastatin tab 20 mg, Amlodipine tab 5 mg and Metformin tab 500 mg respectively. All leftover medicine would be carried and replaced by the new drug. 178 patients indicated the drug-related problems which the highest problem was unused medicine in patients for 64.04 percent, forgetting to take the medicine with 12.92 percent, self-adjustment of medication doses with 5.62 percent, getting drug side effects or medication error 3.37 percent. The result of patient’s satisfaction evaluation found that the patients preferred to get the dispensing by the pharmacist at their house 76.09
percent, while 23.91 percent of the patients were not assured and preferred to get the dispensing at hospital. It was found that the dispensing at patient’s house by the pharmacist resulted in the patients got the correct drug instruction and understand medicine taking correctly. The drug-related problems were resolved, including the price of drug using was decreased, obviously. The data should be forwarded to the other medical officers for acknowledgement and solving the drug-related problems continually.
