Evaluation of the Medication Reconciliation in an Inpatient Unit at Phayu Hospital in Phayu District, Sisaket Province

Abstract

The objective-based evaluation study aimed to evaluate medication reconciliation in an inpatient unit at Phayu hospital in Phayu district, Sisaket province. The sample included patients with chronic disease in the service area of Phayu Hospital, who received treatment between 1 January 2022 and 30 June 2022, a total of 352 patients for 6 months. The data were collected by using a medication reconciliation form. Descriptive statistic was used in this study.

The result found that most of the samples were female (69.89 %), age between 61-80 years old (44.89 %), have hypertension disease (43.47 %). The medication error was higher than the indicators. Medication errors during hospitalizations were 9.94%. Most of the problems were patients not receiving the medication they were supposed to be taking (5.11 %) and changing prescriptions (3.41 %). Medication errors while the patient was discharged or referred to another found up to 12.78 %. Most found prescribing drugs that patients had never received before (7.10 %) and did not receive the same medication they should have received (3.41 %). Most of the two stages were B level in terms of severity level, which was a level that was not dangerous. Especially in the discharge process, therefore the severity levels C and D were lower than that in the hospitalization process. In conclusion, medication reconciliation improves the drug's safety but requires continuity and management in the process where there is a high proportion of problems.
