Development of Medication Home Visit System for warfarin patients whose outside target International Normalized Ratio (INR) of Kantharalak Hospital, Sisaket Province.

The purpose of this action research was to study 1. Develop a Medication home visit system for warfarin patients whose International Normalized Ratio (INR) not in the target criteria by creating a structured medication home visit record form linking of Electronic Medical Record (EMR) Systems “HosXP”. 2. Evaluate the outcome of quantitative and qualitative home visits, using an action research approach in 4 phases: 1. Planning by focus group 2. Implementation: medication home visits and data recording in the system 3. Observation of the implementation and any problem 4. Reflection and summarization of results for further development. Data collected include: Purposive Sampling 51 participate case between August 2024 to February 2025. Data analysis using descriptive statistics.
Study outcomes: Development of medication home visit data entry in HosXP using the “SOAP” note format; satisfaction from multidisciplinary team; 100% regarding to the POP-UP alert system, 66.6% regarding to the data. Quantitative results: Among 51 patients studied, problems with following by the PCNE Ver.9.1 guideline were found: Treatment ineffective concerns in 37 patients (72.6%), Safety concerns in 1 patient (1.9%). Cause of problem leading to; Inappropriate Drug use in 19 patients (37.3%), Problem-solving addressing by; patient-specific issues in 25 patients (49.0%), network level issues in 23 patients (45.1%). Acceptance of a solution; full compliance with treatment plan in 15 patients (29.4%). Qualitative result: After home visits, 31 patients (60.8%) achieved target INR levels. Therefore, it can be concluded that the development of the Medication home visit system has resulted in an appropriate data format in the medical record program, the multidisciplinary team can easily access the data, increase outcomes, and network has the knowledge to provide continuous care.
