บทคัดย่อ

ภาวะมวลกล้ามเนื้อน้อยทุติยภูมิ (Secondary sarcopenia) ภาวะสูญเสียมวลและสมรรถภาพของกล้ามเนื้อจากโรคเรื้อรัง โดยเฉพาะโรคเบาหวานชนิดที่ 2 เมื่อเกิดร่วมกันเรียกว่า Sarcopenic diabetes ทั้งสองเป็นเหตุและผลซึ่งกันและกันก่อให้เกิดความผิดปกติทางเมตาบอลิซึม ส่งผลต่อภาวะทุพพลภาพ คุณภาพชีวิต และภาระทางสาธารณสุข โดยเฉพาะในเอเชียตะวันออกเฉียงใต้ การทบทวนนี้มีเป้าหมายเพื่อสังเคราะห์หลักฐานด้านกลไกพยาธิสรีรวิทยา ความชุก ปัจจัยเสี่ยง เกณฑ์การวินิจฉัยและแนวทางรักษา โดยสืบค้นจากฐานข้อมูลระหว่างปี ค.ศ. 2010–2025 และคัดเลือกบทความที่เกี่ยวข้องจำนวน 30 การศึกษา วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา

หลักฐานส่วนใหญ่พบว่ากลไกสำคัญประกอบด้วยภาวะดื้ออินซูลิน ความผิดปกติของไมโตคอนเดรีย การอักเสบเรื้อรัง และการสะสมของสารเร่งความชรา  ภาวะมวลกล้ามเนื้อน้อยในผู้ป่วยเบาหวานรวมถึงการแทรกซึมของไขมันในกล้ามเนื้อที่สัมพันธ์กับการควบคุมระดับน้ำตาลที่แย่ลง ความชุกของภาวะมวลกล้ามเนื้อน้อยอยู่ที่ร้อยละ 10–40 ในประชากรทั่วไป (เฉลี่ยประมาณร้อยละ 25) ประมาณร้อยละ 18 ในผู้ป่วยเบาหวาน และสูงถึงร้อยละ 37.46 ในเอเชีย โดยความแปรปรวนขึ้นกับเกณฑ์วินิจฉัย และปัจจัยเสี่ยงสำคัญต่อภาวะมวลกล้ามเนื้อน้อยทุติยภูมิ ได้แก่ อายุที่เพิ่มขึ้น การควบคุมระดับน้ำตาลไม่ดี ระยะเวลาเป็นเบาหวานนาน ภาวะแทรกซ้อน และภาวะทุพโภชนาการ  เกณฑ์ที่ใช้คือ AWGS 2019 และ EWGSOP2 แนวทางการดูแล ได้แก่ การออกกำลังกายแบบแรงต้านร่วมกับแอโรบิก การเสริมโปรตีนและวิตามินดี การควบคุมระดับน้ำตาล และการเลือกใช้ยาต้านเบาหวานเฉพาะบุคคล

ภาวะมวลกล้ามเนื้อน้อยทุติยภูมิและ Sarcopenic diabetes เป็นปัญหาสาธารณสุขที่ควรได้รับการคัดกรองและจัดการโดยเน้นการดูแลแบบบูรณาการ เพื่อเพิ่มคุณภาพชีวิตและลดภาระทางเศรษฐกิจในระยะยาว
