ประสิทธิภาพของการตรวจวินิจฉัยวัณโรคและความไวต่อยาด้วยวิธี Line Probe Assay และวิธีมาตรฐานในเขตพื้นที่รับผิดชอบของสำนักงานป้องกันควบคุมโรคที่ 9 จังหวัดนครราชสีมา

เชื้อวัณโรค (Mycobacterium tuberculosis) เป็นปัญหาด้านสาธารณสุขที่สำคัญในประเทศไทย โดยเฉพาะ Multidrug-resistant tuberculosis (MDR-TB) เชื้อวัณโรคที่ดื้อต่อยาไรแฟมพิซิน (RIF) และยาไอโซไนอะซิด (INH) โดยทั่วไปการตรวจ MDR-TB อาศัยการเพาะเลี้ยงเชื้อและทดสอบความไวต่อยาต้านวัณโรคด้วยอาหารชนิดแข็งและอาหารชนิดเหลว ซึ่งการรายงานผลตรวจต้องใช้ระยะเวลานาน สำนักงานป้องกันควบคุมโรคที่ 9 จังหวัดนครราชสีมา ได้นำวิธี Line Probe Assay (LPA) สำหรับตรวจวินิจฉัยเชื้อวัณโรคและการดื้อยา เพื่อเพิ่มความรวดเร็วในการรายงานผล การศึกษานี้เป็นการศึกษาย้อนหลังเชิงพรรณนา ดำเนินการรวบรวมข้อมูลจากแบบบันทึกผลตรวจวิเคราะห์ทางห้องปฏิบัติการ สำนักงานป้องกันควบคุมโรคที่ 9 จังหวัดนครราชสีมา ระหว่างเดือน ตุลาคม พ.ศ. 2562 ถึงเดือน กันยายน พ.ศ. 2563 จำนวน 335 ตัวอย่าง วัตถุประสงค์เพื่อประเมินประสิทธิภาพการตรวจวินิจฉัยเชื้อวัณโรคดื้อยาด้วยวิธี LPA โดยชุดทดสอบ Genotype MTBDRplus VER 2.0 เปรียบเทียบกับผลการทดสอบความไวต่อยาของเชื้อวัณโรคด้วยวิธีมาตรฐาน วิเคราะห์ข้อมูลหาค่าความไว ค่าความจำเพาะ ค่าความถูกต้อง ค่าทำนายผลบวก และค่าทำนายผลลบ ผลการศึกษา พบว่า การทดสอบความไวต่อยาด้วยวิธีมาตรฐาน พบเชื้อ MDR-TB จำนวน 25 ตัวอย่าง (ร้อยละ 7.5) เชื้อที่ดื้อต่อยา INH ชนิดเดียว จำนวน 25 ตัวอย่าง (ร้อยละ 7.5) และไวต่อยาทั้ง 2 ชนิด จำนวน 285 ตัวอย่าง (ร้อยละ 85) ส่วนวิธี LPA สามารถวินิจฉัยเชื้อ MDR-TB จำนวน 21 ตัวอย่าง (ร้อยละ 6) เชื้อที่ดื้อต่อยา INH ชนิดเดียว จำนวน 27 ตัวอย่าง (ร้อยละ 8) เชื้อดื้อต่อยา RIF ชนิดเดียว จำนวน 2 ตัวอย่าง (ร้อยละ 1) และไวต่อยาทั้ง 2 ชนิด จำนวน 285 ตัวอย่าง (ร้อยละ 85) พบว่า การตรวจหาการดื้อยา INH, RIF และชนิด MDR-TB ด้วยวิธี LPA โดยชุดทดสอบ Genotype MTBDRplus VER 2.0 มีความไวร้อยละ 90, 91.7 และ 84 มีความจำเพาะร้อยละ 98.9, 99.3 และ 100 มีค่าความถูกต้องร้อยละ 97.6, 98.8 และ 98.8 มีค่าทำนายผลบวกร้อยละ 93.8, 91.7 และ 100 มีค่าทำนายผลลบร้อยละ 98.3, 99.3 และ 98.7 ตามลำดับ เมื่อเปรียบเทียบกับการทดสอบความไวต่อยาด้วยวิธีมาตรฐาน โดยสรุป LPA ซึ่งเป็นวิธีตรวจทางอณูชีวิทยา เป็นวิธีที่มีความไว ความจำเพาะ และความถูกต้องสูง สามารถนำมาใช้ในการตรวจวินิจฉัยเชื้อวัณโรคและการดื้อต่อยาในตัวอย่างเสมหะส่งตรวจจากผู้ป่วยได้ ลดระยะเวลาในการปฏิบัติงานและการรายงานผลตรวจของห้องปฏิบัติการ ทำให้ผู้ป่วยได้รับการวินิจฉัยและรักษาจากแพทย์ที่รวดเร็ว มีประสิทธิภาพมากขึ้น ช่วยป้องกันการแพร่กระจายของเชื้อวัณโรคจากผู้ป่วยไปสู่ชุมชน
