ความเที่ยงของแบบประเมินแผลเปิดและความสอดคล้องของการประเมิน แผลโดยพยาบาล

บทคัดย่อ

การวัดและประเมินแผลต้องใช้เครื่องมือที่มีความตรง ความเที่ยง สามารถนำไปใช้ประเมินได้จริง และได้ผลลัพธ์เท่าเทียมกันระหว่างผู้ประเมิน การศึกษาเชิงพรรณนาเปรียบเทียบครั้งนี้มีวัตถุประสงค์ เพื่อทดสอบความเที่ยงของแบบประเมินแผลเปิด (the Open WoundAssessment Form: OWAF) และความสอดคล้องของผลการประเมินแผลของทีมพยาบาลในโครงการดูแลผู้ป่วยที่มีทวารเทียมและแผล โรงพยาบาลรามาธิบดี กลุ่มตัวอย่างเป็นแผลชนิดต่าง ๆ จำนวน 31 แผล และพยาบาลวิชาชีพ 4 คน วัดพื้นที่หน้าตัดและความลึกของแผล รวมทั้งประเมินคุณลักษณะต่าง ๆ ของแผล ประกอบด้วย เนื้อแผล ขอบแผล ผิวหนังรอบแผล ซอกใต้ขอบแผล สารคัดหลั่ง และกลิ่น พบว่า การวัดขนาดพื้นที่หน้าตัดและความลึกของแผลมีค่าสัมประสิทธิ์สหสัมพันธ์ภายในกลุ่ม (Intraclass correlation) เท่ากับ .995-.996 และ .998-.999 ตามลำดับ ส่วนการประเมินคุณลักษณะอื่น ๆ มีค่า Kappa .84-1 ผลการศึกษาแสดงว่าแบบประเมินแผลเปิด OWAF เป็นเครื่องมือที่มีความเที่ยงและทีมพยาบาลในโครงการ ฯ มีทักษะในการประเมินแผลเท่าเทียมกัน แบบประเมินแผลเปิดจึงมีความน่าเชื่อถือ แต่เพื่อความถูกต้องเมื่อนำไปใช้ประเมินการเปลี่ยนแปลงของขนาดแผล ต้องจัดท่าให้ผู้ป่วยอยู่ในท่าเดิมเสมอ

Abstract

Wound measurement and assessment need a valid, reliable, and efficient toolas well as agreement between evaluators. This descriptive comparative study aimed toevaluate the reliability of an Open Wound Assessment Form (OWAF) and agreement ofwound measurement and assessment skills using the form among nurses in the Ostomyand Wound Care Team, Ramathibodi Hospital. Four nurses measured the wound areaand wound depth of 31 wounds with different types and locations as well as assessedsix characteristics of the wound including wound bed tissue, wound edge, surroundingskin, undermine, exudate, and ordor. Intraclass correlation coefficients for interratermeasurements were .995-.996 and .998-.999 for wound area and wound depth,respectively. Kappa coefficients for other characteristics of the wounds were .84-1.0.Based on our results, the OWAF was found to have high reliability and the nurses inthe Ostomy and Wound Care Team have equivalent skills for wound measurement andassessment. However, to assess the change of wound areas correctly, the patient’s positionshould be consistent each time when measured.
