Clinical syndrome and pathological diagnosis of Glomerular Diseases in Vachira phuket Hospital

Abstract

             Background: Glomerular disease is a common disease in clinical practice and the leading cause of the end stage renal disease. Most of the patients presented with multiple clinical syndromes which depended on the renal pathology without any epidemiological studies were available in Vachira Phuket hospital and this area. Clinical characteristics and kidney biopsy were important roles for diagnosis and developing appropriate treatment regimens.
              Objective: To study the Correlation between clinical syndrome and the pathologic results of kidney biopsy of the patients with Glomerular disease at the nephro clinic, Vachira Phuket hospital.
              Method: This study was a descriptive retrospective study medical records of patients who were diagnosed with Glomerular disease at nephro clinic, Vachira Phuket hospital between January 2013 and December 2015, were reviewed. Patient’s demographic data, clinical characteristics, details of laboratory tests, renal pathology, 6 months follow up and treatment outcomes were recorded.
              Results: A Total of 65 patients kidney biopsy data were collected during this period. The ratio of female to male was 1.7:1, average age was 41.52 years (SD = 14.36). The patients were primary glomerular disease 52.3% and secondary glomerular disease 47.7%. The most common renal pathological findings were Lupus nephritis (LN), IgA nephropathy (IgAN) and Minimal Change Disease (MCD) accounting for 24.6%, 20% and 13.9%, respectively. The most common clinical syndrome were Nephrotic nephritis syndrome, Nephrotic syndrome and Asymptomatic urinary abnormalities 36.9%, 32.3% and 15.4%, respectively. The most of treatment response show clinical syndrome and renal pathology were Nephrotic nephritis syndrome and Lupus nephritis (LN).The patients whom were poor response to treatment presented with Nephrotic syndrome, IgA nephropathy and diabetic nephropathy.
              Conclusion: The study of clinical characteristics, results of kidney biopsy pathology of Glomerular disease patients in Vachira Phuket Hospital, this study will be apply to the patients’ treatment plan effectively in the area. The follow up and data analyzed should be done to improve and enhance clinical outcomes within this group.
