Abstract

Background: Misoprostol can be administered by either the sublingual or vaginal route for pregnancy termination. However, evidence comparing these two routes in Thailand remains limited. This study compared the effectiveness, safety, and patient satisfaction of sublingual versus vaginal misoprostol for pregnancy termination before 20 weeks of gestation.

Objective: To compare complete abortion rates, time to abortion, requirement for uterine evacuation, and safety between sublingual and vaginal misoprostol.

Methods: A single-center, parallel-group randomized controlled trial was conducted at Chumphon Khet Udomsak Hospital, Thailand, between June 2025 and May 2026. Eighty-eight women were randomly assigned (1:1) to receive misoprostol by either the sublingual or vaginal route.

Results: Complete abortion rates did not differ significantly between the sublingual and vaginal groups (27.3% vs. 22.7%; RR 1.20, 95% CI 0.58–2.48; p = 0.806). Median time to abortion was 11.0 and 17.0 hours, respectively (p = 0.114). The requirement for uterine evacuation, safety outcomes, and patient satisfaction were also comparable between the two groups.

Conclusion: Sublingual and vaginal misoprostol demonstrated comparable effectiveness, safety, and patient satisfaction for pregnancy termination before 20 weeks of gestation. Either route may be used according to clinical indications, patient preference, and healthcare setting.
