ประสบการณ์การผ่าตัดโดยการใช้หนังหุ้มอัณฑะรักษาคนไข้ ที่ฉีดสารเหลวเข้าหนังหุ้มองคชาติ

วัตถุประสงค์ : รายงานการรักษาผู้ป่วยชายที่ฉีดสารแปลกปลอมเข้าไปในอวัยวะเพศชาย (Penile parafinoma or Oleogranuloma or Sclerosing lipogranuloma or Foreign body granuloma)ที่เข้ารับการรักษาตัวในโรงพยาบาลสุราษฎร์ธานี
วิธีการศึกษา : เป็นการศึกษาในช่วงเดือนตุลาคมคม 2554 ถึงเดือนกันยายน 2557 จำนวน 50 ราย ทุกรายมาด้วยภาวะแทรกซ้อน ที่พบบ่อยคือ อักเสบติดเชื้อ, พังผืดรัดองคชาต (fibrosis), อวัยวะเพศแข็งตัวไม่ดี มีปัญหาเวลาร่วมเพศ ซึ่งได้ทำการรักษาโดยการผ่าตัดเลาะเอาผิวหนังและเนื้อเยื่อใต้ผิวหนังออกทั้งหมด (Excision foreign body
granuloma) แล้วปลูกถ่ายผิวหนังใหม่จากหนังหุ้มอัณฑะ โดยวิธี Penoscrotal flap and V-Y flap release technique
ผลการรักษา : อายุเฉลี่ยที่มารับการผ่าตัด 28.5 ปี ฉีดมานานเฉลี่ย 2.5 ปีใช้เวลาผ่าตัดเฉลี่ย 100นาที พบ 4 ราย(8%) มีการติดเชื้อบริเวณแผลที่เย็บ(wound infection),
5 ราย(10%) มีปัญหาแผลแยก(wound dehiscene), 4 ราย (8%) มีปัญหาแผลคล้ายไฟลวกบริเวณรอยเย็บ(skin maceration),ไม่พบมีรายใดมีปัญหาเรื่อง skin necrosis, Decrease
of sexual satisfaction, Electile dysfuction, Penile shortening และเมื่อนัดดูแผลพบว่าบาดแผลจากการทำผ่าตัดแห้งดีไม่มีการติดเชื้อและพบว่าทุกรายพอใจผลการผ่าตัด ไม่มีใครมารับการผ่าตัดซ้ำหรือผ่าตัดเพิ่มเติม ผลการผ่าตัดดีเป็นที่น่าพอใจของแพทย์และผู้ป่วย
สรุป : การผ่าตัดแก้ไข ชายที่ฉีดสารแปลกปลอมเข้าไปในอวัยวะเพศโดยวิธี Excision foreign body granuloma with Penoscrotal flap and V-Y flap release technique (One Stage)ในกรณีที่ผู้ป่วยมีหนังหุ้มอัณฑะเหลือมากพอที่จะนำมาใช้แทนส่วนที่เลาะออกไป ซึ่งได้ผลดีมาก ทำไม่ยาก ใช้เวลาไม่นาน มีภาวะแทรกน้อยมากและผู้ป่วยไม่ต้อง
เสียเวลามาทำผ่าตัดหลายครั้ง/การใช้หนังหุ้มอัณฑะจะได้หนังหุ้มองคชาติที่มีลักษณะใกล้เคียงกับของเดิมให้ความรู้สึกเมื่อได้รับการสัมผัสเหมือนปกติ สามารถแข็งตัวและสามารถใช้ในการร่วมเพศได้ดีเหมือนปกติ
