แบบสำรวจแนวทางการรักษาโรคไทรอยด์เกรฟส์ในเวชปฏิบัติปี 2564 จังหวัดสุราษฎร์ธานี

วัตถุประสงค์: เพื่อศึกษาแนวทางการวินิจฉัยและรักษาโรคไทรอยด์เกรฟส์ในเวชปฏิบัติที่จังหวัด
สุราษฎร์ธานีในปี 2564 เพื่อนำไปใช้เป็นแนวทางในการวางแผนพัฒนาระบบการดูแลผู้ป่วย
วิธีการศึกษา: ศึกษาวิจัยข้อมูลแบบภาคตัดขวาง โดยการส่งแบบสอบถามอิเล็กทรอนิกส์จำนวน 33 ข้อ ให้แพทย์ผู้รักษาโรคไทรอยด์เกรฟส์ที่ทำงานในจังหวัดสุราษฎร์ธานีตอบแบบสอบถามตั้งแต่เดือนกุมภาพันธ์ 2564 ถึงเดือนมิถุนายน 2564
ผลการศึกษา: มีแพทย์ผู้เข้าร่วมตอบแบบสอบถามจำนวน 205 คน การวินิจฉัยโรคไทรอยด์เกรฟส์นิยมเลือกการเจาะเลือดเพื่อดูค่าการทำงานของฮอร์โมนไทรอยด์มากกว่าการตรวจทางเอกซเรย์ โดยนิยมตรวจระดับเลือดทีเอสเอช 99% และฟรีไทรอยด์ฮอร์โมน 91.7% มากเป็นสองอันดับแรก สำหรับการรักษาโรคเลือกการใช้ยาต้านไทรอยด์ฮอร์โมน (92.2%) มากกว่าการกลืนแร่ (3.9%) และการผ่าตัด (2.0%) โดยยาต้านไทรอยด์ที่ใช้มากที่สุดคือยาเมธิมาโซล โดยใช้ขนาดยาเริ่มต้น 15 มิลลิกรัมต่อวันร่วมกับยาเบต้าบล็อกเกอร์ การรักษาโรคไทรอยด์เกรฟส์ขณะตั้งครรภ์พบว่าเลือกใช้ยาต้านไทรอยด์เป็นอันดับแรก โดยยาที่ใช้คือโพรพิลไทโอยูราซิลมากกว่ายาเมธิมาโซล โดยเฉพาะในสามเดือนแรกของการตั้งครรภ์
สรุป: แนวทางการรักษาโรคไทรอยด์เกรฟส์ของจังหวัดสุราษฎร์ธานีนิยมใช้ยาต้านไทรอยด์ฮอร์โมนรักษาเป็นทางเลือกแรกมากกว่าการกลืนแร่และการผ่าตัด แนวทางการรักษาเหมือนทางประเทศในแถบอเมริกา ยุโรปและเอเชีย นอกจากนั้นการรักษาโดยการกลืนแร่หรือการผ่าตัดขึ้นกับประสบการณ์ของแพทย์แต่ละท่านและความพร้อมของโรงพยาบาล
