บทคัดย่อ

บทนำ: ฟันแท้หลุดจากเบ้าเป็นการบาดเจ็บทางทันตกรรมในเด็กที่ต้องรักษาอย่างเร่งด่วนตามแนวทาง International Association of Dental Traumatology (IADT) เพื่อลดภาวะแทรกซ้อนที่อาจเกิดขึ้น

รายงานผู้ป่วย: รายงานฉบับนี้นำเสนอผู้ป่วยเด็ก 2 รายที่ได้รับการปลูกฟันคืนลงในเบ้าฟันภายหลังอุบัติเหตุฟันหลุดจากเบ้า โดยรายที่ 1 เป็นเด็กหญิงอายุ 9 ปี ฟันแท้หน้าบนซี่ 21 และรายที่ 2 เป็นเด็กชายอายุ 8 ปี ฟันแท้หน้าบนซี่ 11 หลุดจากเบ้า ผู้ปกครองของทั้งสองรายเก็บฟันไว้ในนมและนำมาพบทันตแพทย์ภายใน 20 และ 30 นาทีตามลำดับ ภายหลังการตรวจทันตแพทย์ทำความสะอาดฟันด้วยน้ำเกลือปราศจากเชื้อและปลูกฟันคืนในเบ้าฟันเดิม จากนั้นเข้าเฝือกฟันแบบยืดหยุ่นเป็นเวลา 2 สัปดาห์ ให้ยาปฏิชีวนะ และเริ่มรักษาคลองรากฟันภายใน 1 สัปดาห์ การติดตามแสดงให้เห็นผลลัพธ์ที่ดีที่ระยะ 3 ปีในรายที่ 1 และ1.5 ปี ในรายที่ 2 พบว่าฟันมีความมั่นคง เนื้อเยื่อเหงือกอยู่ในสภาพปกติ โดยรายที่ 1 มีสีฟันเข้มขึ้นเล็กน้อย ภาพถ่ายรังสีของทั้งสองรายแสดงช่องเอ็นยึดปริทันต์ต่อเนื่อง ไม่พบการละลายของรากฟันหรือภาวะฟันยึดแข็ง

สรุป: การจัดการตามแนวทาง IADT สามารถคงสภาพฟันแท้ที่หลุดจากเบ้าได้อย่างมีประสิทธิภาพ แม้ฟันอยู่นอกช่องปากถึง 30 นาที ดังนั้นการให้ความรู้แก่ผู้ปกครองและครูเกี่ยวกับการปฐมพยาบาลฟันหลุดจากเบ้าจึงมีความสำคัญอย่างยิ่ง
