Siliconoma: A case report

Silicone has been widely used in medicine for several decades especially for permanent soft tissue augmentation. Although it is considered as biologically inert, a number of individuals experienced various reactions. Some of delayed reactions include a foreign body reaction, granuloma, and migration of the substance. Granuloma may occur from a few weeks after the injection to several decades afterward and can be rigorously debilitating, adversely affecting quality of life. Clinically, silicone granuloma may present as erythematous papules or plaques, painful nodules, diffuse swelling, cellulitis-like, and in some cases, ulceration, migration or palpable lymphadenopathy. Histologic findings of a biopsy specimen displayed granulomatous inflammatory infiltrate of lymphocytes, histiocytes and multinucleated giant cells surrounding an exogenous material with numerous “Swiss-cheese” like cystic spaces of varying sizes in the dermis. We report a patient who presented with multiple facial siliconomas and emphasize on clinical presentation, therapeutic approach, and favorable outcomes with combination of systemic and intralesional corticosteroid administration.
