Chronic erosive HSV infection in HIV-infected patient after highly active antiretroviral therapy (A possible immune reconstitution syndrome)

Abstract

A twenty-year-old female patient presented with multiple ulcers of toe, sole, thigh and buttock for five months. At that time, diagnosis was herpes simplex virus infection and acyclovir therapy was started. These ulcers were resolved except the lesions at genitalia and buttock after highly active antiretroviral therapy.  The high dose and prolong acyclovir administration was failed to heal, the immune reconstitution syndrome was suspected. Finally, we tried to treat her with systemic corticosteroid and oral acyclovir. After one week, the lesions were improved markedly and all medications were discontinued two weeks later, except antiretroviral therapy.
