บทนำ
อุบัติเหตุทางถนนเป็นสาเหตุสำคัญของการเสียชีวิตและความพิการในเด็กและวัยรุ่น โดยเฉพาะในกลุ่มอายุ 5-25 ปี จากรายงานสถิติอุบัติเหตุทางถนน ปี พ.ศ. 2564 พบว่ากลุ่มอายุ 15-19 ปี เป็นกลุ่มที่มีการบาดเจ็บและเสียชีวิตมากที่สุด คิดเป็นร้อยละ 22.0 การระบุปัจจัยที่สัมพันธ์กับการเสียชีวิตในระยะเริ่มต้นอาจช่วยสนับสนุนการประเมินความเสี่ยงและการดูแลผู้ป่วยบาดเจ็บได้อย่างมีประสิทธิภาพมากขึ้น
วัตถุประสงค์
เพื่อศึกษาปัจจัยที่สัมพันธ์กับการเสียชีวิตภายใน 24 ชั่วโมงของผู้ป่วยเด็กและวัยรุ่นที่ได้รับบาดเจ็บจากอุบัติเหตุจราจรและเข้ารับการรักษา ณ ห้องฉุกเฉิน โรงพยาบาลขอนแก่น
วิธีการศึกษา
การศึกษานี้เป็นการศึกษาย้อนหลังแบบ case-control study ในผู้ป่วยอายุ 0-19 ปี ที่ได้รับบาดเจ็บจากอุบัติเหตุจราจรและเข้ารับการรักษา ณ ห้องฉุกเฉิน โรงพยาบาลขอนแก่น ระหว่างเดือนมกราคม พ.ศ. 2557 ถึงเดือนธันวาคม พ.ศ. 2564 โดยคัดเลือกเฉพาะผู้ป่วยที่ได้รับการคัดแยกระดับ 1 ซึ่งเป็นผู้ป่วยวิกฤติ แบ่งเป็นกลุ่มผู้เสียชีวิตภายใน 24 ชั่วโมง จำนวน 144 ราย และกลุ่มผู้รอดชีวิต จำนวน 144 ราย
ผลการศึกษา
ผู้ป่วยทั้งหมด 288 ราย ได้รับการคัดแยกระดับ 1 เป็นผู้ป่วยวิกฤติ ในจำนวนนี้เป็นผู้เสียชีวิตภายใน 24 ชั่วโมง 144 ราย และผู้รอดชีวิต 144 ราย กลุ่มผู้เสียชีวิตส่วนใหญ่เป็นเพศชาย 115 ราย (ร้อยละ 79.9) และช่วงอายุที่พบการเสียชีวิตมากที่สุดคือ 15-19 ปี จำนวน 104 ราย (ร้อยละ 72.2) ภายหลังการควบคุมตัวแปรกวน พบว่าปัจจัยที่สัมพันธ์กับการเสียชีวิตภายใน 24 ชั่วโมงอย่างมีนัยสำคัญทางสถิติ ได้แก่ ภาวะความดันโลหิตต่ำแรกรับที่ห้องฉุกเฉิน (AOR 182.11; 95% CI 20.00-1657.48; p < 0.001) การบาดเจ็บบริเวณทรวงอก (AOR 4.94; 95% CI 1.87-13.05; p = 0.001) ระดับความรู้สึกตัว Glasgow Coma Scale (GCS) 3-8 คะแนน แรกรับที่ห้องฉุกเฉิน (AOR 96.08; 95% CI 4.48-2057.49; p = 0.003) และระดับความรุนแรงของการบาดเจ็บ AIS >3 (AOR 3.26; 95% Cl 1.11-9.57; p = 0.031)
