บทคัดย่อ

บทนำ
แผนกฉุกเฉินเป็นจุดรับบริการสำคัญที่มีบทบาทในการระบุผู้ป่วยระยะท้ายเพื่อเข้าสู่ระบบการดูแลแบบประคับประคอง แต่ด้วยข้อจำกัดด้านเวลาและทรัพยากร การเลือกใช้เครื่องมือคัดกรองที่เหมาะสมจึงเป็นเรื่องท้าทาย
วัตถุประสงค์
เพื่อเปรียบเทียบความแม่นยำเชิงพยากรณ์ของแบบคัดกรอง Supportive and Palliative Care Indicators Tool (SPICT), Modified Surprise Question (MSQ) และ Palliative Care and Rapid Emergency Screening (P-CaRES)ในการทำนายการเสียชีวิตภายใน 3 เดือน และประเมินความสอดคล้องระหว่างเครื่องมือคัดกรองทั้งสามชนิด
วิธีการศึกษา
เป็นการศึกษาเชิงพยากรณ์แบบติดตามไปข้างหน้าในกลุ่มตัวอย่าง 281 ราย วิเคราะห์ความแม่นยำด้วยค่าสถิติ C-statistic อัตราส่วนอันตรายที่ปรับแล้ว (Adjusted Hazard Ratio: aHR) และเปรียบเทียบการรอดชีวิตด้วย Kaplan-Meier
ผลการศึกษา
กลุ่มตัวอย่างเสียชีวิตภายใน 3 เดือน จำนวน 158 ราย (ร้อยละ 56.23) เครื่องมือทั้ง 3 ชนิดทำนายการเสียชีวิตได้อย่างมีนัยสำคัญทางสถิติ (p<0.001) โดย P-CaRES มีความแม่นยำสูงสุด (C-statistic 0.6666; 95% CI: 0.60-0.73) และระบุกลุ่มเสี่ยงที่มีมัธยฐานการรอดชีวิตสั้นที่สุดคือ 17 วัน (95% CI: 11.92-22.08) ขณะที่ MSQ ให้ค่าอัตราส่วนอันตรายที่ปรับแล้ว สูงที่สุดคือ 2.59 (95% CI: 1.78-3.76; p<0.001) สำหรับความสอดคล้องระหว่างเครื่องมืออยู่ในระดับพอใช้ (Fair agreement) โดยเครื่องมือ SPICT กับ P-CaRES มีความสอดคล้องสูงสุด (kappa = 0.374; p<0.001)
