บทนำ
การเตรียมองศาการดัดลวดนำในท่อช่วยหายใจ เป็นปัจจัยสำคัญต่อความรวดเร็วในการใส่ท่อช่วยหายใจ อย่างไรก็ตาม ปัจจุบันยังไม่มีแนวปฏิบัติทางคลินิกที่ระบุถึงองศาความโค้งที่เหมาะสมที่สุดสำหรับการใส่ท่อช่วยหายใจ
วัตถุประสงค์
เพื่อศึกษาองศาการดัดลวดนำในของท่อช่วยหายใจต่อระยะเวลาการใส่ท่อช่วยหายใจ ผลต่ออัตราการใส่ท่อช่วยหายใจสำเร็จภายในครั้งแรก และอัตราล้มเหลวในการใส่ท่อช่วยหายใจ
วิธีการศึกษา
การทดลองแบบสุ่มแบบไขว้ในสถานการณ์จำลองเปรียบเทียบองศาลวดนำในท่อช่วยหายใจต่อระยะเวลาการใส่ท่อช่วยหายใจ โดยบุคลากรทางการแพทย์ระดับต่าง ๆ จำนวน 70 คน ที่โรงพยาบาลสระบุรี ตั้งแต่วันที่ 1 มกราคม ถึง 31 ธันวาคม พ.ศ. 2568โดยใส่ท่อช่วยหายใจตามชุดลำดับองศาที่กำหนดวัดผลเป็นระยะเวลาและจำนวนครั้งในการใส่ท่อช่วยหายใจ
ผลการศึกษา
การใส่ท่อช่วยหายใจด้วยองศาการดัดลวดน้ำที่มุม 15 องศามีระยะเวลาเฉลี่ยปรับใช้ (adjusted mean time [aMT] เท่ากับ 18.2 วินาที (9596 confidence interval [CI], 15.9-20.4) การใส่ท่อช่วยหายใจด้วยองศาการดัดลวดนำที่มุม 30 และ 45 องศา ใช้เวลาน้อยกว่ามุม 15 องศาอย่างมีนัยสำคัญ (aMT = 15.4;9596CI, 13.1-17.6; p = 0.03 และ aMT = 15.5; 9596CI, 13.2-17.8; 0.04 ตามลำดับ) และไม่มีอัตราล้มเหลวในการใส่ท่อช่วยหายใจ ในขณะที่มุม 75 และ 90 องศาใช้เวลามากกว่ามุม 15 องศา (aMT = 21.1; 9596Cl, 18.8-23.3; p = 0.03 และ aMT = 26.0;95%Cl, 23.7-28.2; p < 0.01 ตามลำดับ) และมีอัตราล้มเหลวที่ ร้อยละ 1.4 ส่วนอัตราการใส่ท่อช่วยหายใจสำเร็จภายในครั้งแรกของทุกมุมไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ
สรุป
การใส่ท่อช่วยหายใจด้วยองศาการดัดลวดนำที่มุม 30 ถึง 45 องศา ใช้เวลาในการใส่ท่อช่วยหายใจน้อยที่สุดในหุ่นจำลอง เมื่อเปรียบเทียบกับการใช้มุมอื่น ๆ ทุกองศาที่แตกต่างกันไม่มีผลต่ออัตราความสำเร็จในการใส่ท่อช่วยหายใจครั้งแรก
