Comparison for Efficacy in Postoperative Pain Relief in Major Gynecologic Surgery of Buprenorphine and Meperidine

Objective: To compare the efficacy of sublingual buprenorphine versus intravenous meperidine in major gynecologic surgery.
Study design: Gynecology ward, Department of Obstetrics and Gynecology, BMA Medical College and Vajira Hospital.
Subjects: Sixty six patients who were operated for gynecologic condition between June 1999 and May 2000.
Methods: All 66 patients were divided into 2 groups by random allocation. Group 1 (n=33) were received sublingual buprenorphine 0.4 mg due to pain after the surgery. Group 2 (n=33) were received intravenous meperidine 1mg/kg due to pain after the surgery. Pre and post operative pain score were evaluated by visual analogue scale (VAS).
Main outcome measures: Mean pain relief of both groups.
Results: The mean pain relief at 4 hour post operation in the buprenorphine group was statistically significant higer than the meperidine group, 8.1+1.93 and 3.8+3.44 respectively (95% CI 3.0 to 5.7, p < 0.001). The number of patients requiring second dose in buprenorphine group was less than of meperidine group (4(12.1%) and 18(54.5%), respectively) which was statistically significant difference (p = 0.001). The side effects between the two groups were no statistically significant difference (p > 0.05).
Conclusion: Sublingual buprenorphine was better than meperidine in postoperative pain relief in major gynecologic surgery.
