External root resorption can occur from many reasons and requires the proper management.  This study presents a case report of external root resorption due to pulp necrosis.  A 20-year-old male experienced pain and mobilization of his upper right central incisor.  After proper diagnostic procedure, the incisal edge was reduced to prevent further trauma.  Then the root canal was treated and filled with calcium hydroxide. This treatment was intervally repeated every three months for one year.  After that, the presented symptoms disappeared and the periapical film showed good healing of periapical bone.  Finally, the root canal was filled with gutta-percha.  At the 6-month and 12-month after filling, both clinical and radiographic examinations revealed complete healing.
