This is an action research. The objectives of the research were to study The Quality Management Model of Primary Care Cluster (PCC) by Community Participation Mechanism for Primary Care Cluster Standard at Non Sawang Health Promoting Hospital,Si Songkhram Sub-District ,Wang Saphung District, Loei Province. The target group are those involved in Primary Care Cluster by order to appoint a Primary Care Cluster committee, a total of 50 people, collecting both quantitative and qualitative data. Data were analyzed by using percentage, mean, standard deviation. Compared between before and after development of the model with Paired t-test dependent statistic and content analysis. The results showed that operational satisfaction and participation in operations of target group between before development were significantly different from after development with statistically significant different at 95% confidence level. This action research can give the model of development of a Quality Management Model of Primary Care Cluster by Community Participation Mechanism for Primary Care Cluster Standard at Non Sawang Health Promoting Hospital Si Songkhram Sub-District Wang Saphung District, Loei Province by Participation Mechanism for Primary Care Cluster (PCC). Conclusion, the succeed of the development of a Quality Management Model of PCC by Community Participation Mechanism for to be Primary Care Cluster Standard Occurred by the participation mechanism of the Primary Care Cluster team from all of sector. Including; the participation decision, the Participatation in the operated working, the Participation in receiving benefits and the participatation in the evaluation
