This research aimed to develop and evaluate the effectiveness of a community participatory health promotion and rehabilitation model for transitioning homebound elderly to socially engaged elderly, called the ACTIVE Model. The study employed a Research and Development (R&D) design consisting of four phases: 1) studying the situation and needs of homebound elderly and the community; 2) developing the model based on Pender’s Health Promotion Model (HPM); 3) implementing the model with 30 homebound elderly participants in Donyanang Subdistrict, Nong Bua Lam Phu Province, for 16 weeks; and 4) evaluating and refining the model. Research instruments included the Barthel Activities of Daily Living (ADL) Index, The WHOQOL-BREF-THAI quality of life questionnaire and and a questionnaire based on Pender’s Health Promotion Model. Data were analyzed using descriptive statistics and Paired t-test. The results showed that after the intervention, the elderly had significantly higher ADL scores (p < .001), increasing from a mean of 10.53 to 13.27. A total of 73.33% (22 out of 30 participants) transitioned from homebound to socially engaged status. Their overall quality of life was moderate, Scores on Pender’s health promotion factors increased from a mean of 3.22 (SD = 0.51) to 3.84 (SD = 0.43). The findings indicate that the ACTIVE Model is an effective and feasible approach for promoting health and rehabilitating homebound elderly to become socially engaged through sustainable community participation.
