Development of Participatory and Seamless Psychiatric Substance Abuse Care System Lopburi Model: A Case Study of Khok Samrong District

Abstract
This research is a development-oriented study aimed at improving the mental health and substance abuse patient care system through participatory and seamless approaches in the Khok Samrong district, Lopburi province. The development process consists of four phases: 1) analysis of the problem situation, 2) model development, 3) implementation, and 4) outcome evaluation. The study was conducted from July to October 2016, with two sample groups: 217 participants involved in the development (community mental health network) and 95 participants in the outcome evaluation (caregivers and patients). Research tools included questionnaires and data recording forms. Statistical analysis involved percentages, means, standard deviations, and paired samples t-tests. The development process comprised five activities: 1) network establishment, 2) collaborative brainstorming, 3) health services provision, 4) positive reinforcement, and 5) risk monitoring. Results showed that after network development, knowledge significantly increased from 42.4% to an average score of 50.7. Participation also increased from 3.34±1.01 to 3.39±1.06 (p<0.001). Network participation increased from 2.80±0.96 to 3.88±0.97. Caregiver burden decreased from 3.48±0.61 to 2.85±0.79. Knowledge significantly improved from 76.8% to 88.4, and the ability to care for patients increased from 3.34±0.79 to 3.74±0.94. Post-development patient care outcomes showed self-care symptoms at 85.3%, and daily routine abilities at 48.4%. In conclusion, the development of the mental health and substance abuse patient care system involved implementing a comprehensive care and mental health promotion system in the community. Family caregivers engaged in home visits, acting as a support mechanism for the complete care of the patient, resulting in improved daily functioning for the patients.
