Anesthetic Management in Patient Under Investigation (PUI) for COVID-19 Undergoing Emergency Tracheostomy in Songklanagarind Hospital: A Case Report

Background: Healthcare personnel is at risk of infection
during the 2019 novel coronavirus disease (COVID-19)
pandemic especially working in the aerosol-generating
procedures. Infection control and prevention are essential
during the perioperative period and should be implemented
to prevent the outbreak within healthcare facilities.
Case report: A 68-year-old Thai man who resides in the
area with ongoing outbreaks of COVID-19. He is a known
case of squamous cell carcinoma of the tongue, chronic
kidney disease, ischemic stroke and history of paroxysmal
atrial fibrillation, presented with pneumonia, and impending
upper airway obstruction. Emergency awake tracheostomy
was done cautiously following the Songklanagarind
hospital’s guideline for emergency tracheostomy in
patients with suspected or confirmed COVID-19. The
operation was uneventful.
Conclusion: Apart from the appropriate anesthetic
technique, multidisciplinary care planning, use standard
and transmission-based precaution, and simulation
training are key considerations to protect healthcare
personnel from exposure.
