คำแนะนำเวชปฏิบัติการบำบัดปวดในหญิงตั้งครรภ์

บทคัดย่อ
การบำบัดปวดในหญิงตั้งครรภ์เป็นประเด็นที่ซับซ้อน เนื่องจากการเปลี่ยนแปลงทางสรีรวิทยาระหว่างตั้งครรภ์และข้อจำกัดด้านความปลอดภัยของการใช้ยา คำแนะนำเวชปฏิบัติฉบับนี้จัดทำโดยสมาคมการศึกษาเรื่องความปวดแห่งประเทศไทย เพื่อเป็นแนวทางเชิงปฏิบัติสำหรับบุคลากรทางการแพทย์ในการประเมินและจัดการความปวดอย่างเหมาะสม ครอบคลุมทั้งอาการปวดที่พบบ่อย เช่น ปวดหลัง ปวดอุ้งเชิงกราน ปวดศีรษะ และอาการปวดเฉียบพลัน รวมถึงการบำบัดปวดในระยะคลอดและหลังคลอด คำแนะนำนี้เน้นการบำบัดปวดด้วยวิธีไม่ใช้ยาเป็นอันดับแรก เช่น การทำกายภาพบำบัด หากจำเป็นต้องใช้ยาควรพิจารณาอย่างรอบคอบ โดยอ้างอิงระบบการจัดกลุ่มความปลอดภัยของยา ได้แก่ Pregnancy and Lactation Labeling Final Rule และ Australian category นอกจากนี้ยังให้ข้อมูลระดับความน่าเชื่อถือของหลักฐานเชิงประจักษ์ และน้ำหนักคำแนะนำ ของการบำบัดปวดวิธีต่างๆอย่างเป็นระบบและชัดเจน เพื่อสนับสนุนการตัดสินใจทางคลินิก อีกทั้งยังเน้นการให้ความรู้และคำปรึกษาแก่หญิงตั้งครรภ์ เพื่อเสริมสร้างความเข้าใจและมีส่วนร่วมในการตัดสินใจเลือกแนวทางการบำบัดปวดที่เหมาะสมที่สุด มุ่งหวังให้เกิดการประยุกต์ใช้จริงในงานบริการสุขภาพของประเทศไทย เพื่อยกระดับคุณภาพการดูแล ลดภาวะแทรกซ้อน และเพิ่มทั้งความปลอดภัยและคุณภาพชีวิตของมารดาและทารกอย่างยั่งยืน
