Comparing Etoricoxib 90 mg. and 120 mg. for postoperative analgesia in arthroscopic anterior cruciate ligament reconstruction

Background: Based on multimodal analgesia, non-opioid analgesics in combination with opioid have widely been a part in pain management. Previous studies have indicated that etoricoxib 120 mg. is effective in reducing postoperative pain.
Objective: The purpose of the present study was to compare the efficacy of etoricoxib 90 mg. with 120 mg. in arthroscopic anterior cruciate ligament reconstruction.
Methods: Sixty patients scheduled for elective arthroscopic anterior cruciate ligament reconstruction under spinal anesthesia were included. They were randomized to receive etoricoxib either 90 mg. (n=30) or 120 mg. (n=30) 1 hour before surgery. A patient-controlled analgesia (PCA) device with intravenous morphine was used postoperatively. 24-hour morphine consumption, pain scores, satisfaction scores and adverse events were recorded.
Results: Baseline characteristics including age, sex, the American Society of Anesthesiologists (ASA) physical status, associated injury, concomitant surgery and duration of surgery were not significantly different between 90 mg. and 120 mg. of etoricoxib group. Median (25th and 75th interquartile range) of total morphine consumption during the first 24 hours was 27 (14-42.5) mg. in the etoricoxib 90 mg. group and 25.5 (18-41) mg. in the etoricoxib 120 mg group (p=0.91). There were no difference in the level of pain intensity score, the level of satisfaction in terms of pain relief and the incidence of adverse events in both groups.
Conclusion: Etoricoxib 90 mg. and 120 mg. showed similar efficacy in the postoperative pain relief after an arthroscopic anterior cruciate ligament reconstruction.
เปรียบเทียบประสิทธิผลของ Etoricoxib 90 มิลลิกรัมกับ 120 มิลลิกรัมต่อการระงับปวดหลังผ่าตัดเอ็นไขว้หน้าข้อเข่าผ่านกล้อง
หลักการและเหตุผล: เทคนิคการระงับปวดที่ใช้ยาที่ไม่ใช่ opioid ร่วมกับ opioid เพื่อบำบัดความปวด หลังผ่าตัด ทำให้ประสิทธิภาพการระงับปวดดีขึ้น การศึกษาก่อนหน้านี้พบว่า etoricoxib 120 มิลลิกรัม มีประสิทธิภาพดีในการระงับปวดหลังผ่าตัด ผู้วิจัยต้องการศึกษาเพื่อเปรียบเทียบประสิทธิภาพการระงับปวด ของ etoricoxib 2 ขนาด คือ 90 และ 120 มิลลิกรัม ในการผ่าตัดเอ็นไขว้หน้าข้อเข่าผ่านกล้อง
วัสดุและวิธีการ: ศึกษาในผู้ป่วย 60 คน ที่นัดมาทำผ่าตัดเอ็นไขว้หน้าข้อเข่าผ่านกล้อง แบ่งเป็น 2 กลุ่มๆ ละ 30 คน โดยการสุ่มให้ ได้รับ etoricoxib ขนาด 90 หรือ 120 มิลลิกรัม 1 ชั่วโมงก่อนผ่าตัด ผู้ป่วยทุกคนได้รับการระงับความรู้สึกโดยการ ฉีดยาชาเข้าช่องน้ำไขสันหลังและได้ยาระงับปวดหลังผ่าตัดเป็น morphine ทางหลอดเลือดดำผ่านเครื่องควบคุม ความปวดด้วยตนเอง บันทึกคะแนนความปวด ภาวะแทรกซ้อนที่เกิดขึ้น คะแนนความพึงพอใจต่อการระงับ ปวดและปริมาณ morphine ที่ใช้ใน 24 ชั่วโมง
ผลการวิจัย: ไม่มีความแตกต่างของข้อมูลพื้นฐานได้แก่ อายุ เพศ ASA physical status ความผิดปกติที่พบในข้อเข่า ชนิดของการผ่าตัดและระยะเวลาผ่าตัด กลุ่มที่ได้ etoricoxib 90 มิลลิกรัม ใช้ morphine เฉลี่ยใน 24 ชั่วโมง 27 (14-42.5) มิลลิกรัม ส่วนกลุ่มที่ได้ etoricoxib 120 มิลลิกรัม ใช้ morphine เฉลี่ย 25.5 (18-41) มิลลิกรัม (p=0.91) ระดับคะแนนความปวด คะแนนความพึงพอใจต่อการ ระงับปวด และภาวะแทรกซ้อนที่เกิดขึ้นไม่มีความแตกต่างกันระหว่างสองกลุ่ม
สรุป: etoricoxib 90 มิลลิกรัม มีประสิทธิภาพในการระงับปวดหลังผ่าตัดเอ็นไขว้หน้าข้อเข่าผ่านกล้อง ไม่แตกต่างจากขนาด 120 มิลลิกรัม
