Ultrasound-guided brachial plexus blocks: a review of techniques and approaches

Background: With the advent of ultrasonography (US), multiple methods have been described for brachial plexus block (BPB). In this review article, we searched all level 1 evidence (randomized trials) published in English in order to determine the optimal approaches and techniques for US-guided BPB.
Objectives: To determine the best techniques and approaches for US-guided BPB.
Methods: The MEDLINE, EMBASE, and PUBMED databases were searched by using the keywords: “brachial plexus, “block, “ultrasound”, “interscalene”, “supraclavicular”, “infraclavicular”, and “axillary”. The search was limited to randomized controlled trials (RCTs) involving human subjects and published in the English language. Only trials comparing techniques and approaches of US-guided BPB were selected.
Results: The search criteria yielded 7 RCTs which compared approaches for US-guided BPB and 14 RCTs which investigated the optimal technique for each approach. The average sample size was 88 subjects. Most trials provided sample size justification, blinded assessment and Jadad scores > 3.
Conclusions: Based on the available evidence, intra-or peri-plexus injection, targeted intracluster injection, perivascular single-injection, and perivascular double-injection constitute the best techniques for interscalene, supraclavicular, infraclavicular and axillary BPB, respectively. When optimal techniques are employed, the supraclavicular, infraclavicular and axillary approaches result in similar efficacy. However, the infraclavicular approach may result in improved efficiency because it requires a single injection dorsal to the axillary artery. Compared to their supraclavicular counterparts, infraclavicular perineural catheters require a shorter performance time and seem to provide better analgesia on the first postoperative day. Current level 1 evidence allows for limited recommendations regarding the optimal approaches and techniques for US-guided BPB. Further RCTs are warranted to tackle unresolved questions.
บทความปริทัศน์ เรื่อง การสกัดกั้นข่ายประสาทของแขนโดยใช้เครื่องคลื่นเสียงความถี่สูงช่วยนำทาง: ทบทวนเทคนิคและบริเวณในการเข้าหาข่ายประสาท
บทนำ: จากความก้าวหน้าของการใช้เครื่องคลื่นเสียงความถี่สูงเพื่อนำทางในการสกัดกั้นข่ายประสาท ของแขน จึงพบความหลากหลายของเทคนิคในการเข้าหาข่ายประสาท ในบทความปริทัศน์ฉบับนี้ผู้ทบทวนงาน วิจัยได้ค้นหาและวิจารณ์หลักฐานที่มีความน่าเชื่อถือ (งานวิจัยเชิงทดลองแบบสุ่ม) ซึ่งตีพิมพ์เป็นภาษาอังกฤษ เพื่อหาวิธีการสกัดกั้นข่ายประสาทของแขนโดยใช้เครื่องคลื่นเสียงความถี่สูงที่เหมาะสม
วัตถุประสงค์: เพื่อระบุ เทคนิคและบริเวณในการเข้าหาข่ายประสาทที่ดีที่สุดสำหรับการสกัดกั้นข่ายประสาทของแขนโดยใช้เครื่อง คลื่นเสียงความถี่สูงช่วยนำทาง
วิธีการศึกษา: ค้นหางานวิจัยในฐานข้อมูล MEDLINE, EMBASE และ PUBMED โดยใช้คำหลัก ดังนี้ “brachial plexus”, “block”, “ultrasound”, “interscalene”, “supraclavicular”, “infraclavicular” และ “axillary” ทั้งนี้จำกัดเฉพาะงานวิจัยที่ทำในมนุษย์และตีพิมพ์เป็นภาษาอังกฤษ จากนั้นคัดเลือกเฉพาะ งานวิจัยที่เปรียบเทียบแต่ละเทคนิค และเปรียบเทียบการเข้าหาข่ายประสาทของแขนบริเวณต่างๆ โดยใช้คลื่นเสียง ความถี่สูงช่วยนำทางเท่านั้น
ผลการศึกษา: จากเกณฑ์การคัดเลือก พบ 7 งานวิจัยที่เปรียบเทียบความแตกต่าง ของการเข้าหาข่ายประสาทที่บริเวณต่างๆ และ 14 งานวิจัยที่เปรียบเทียบความแตกต่างของแต่ละเทคนิคในการ เข้าหาข่ายประสาท ในแต่ละงานวิจัยมีจำนวนขนาดกลุ่มตัวอย่างเฉลี่ย 88 คน โดยงานวิจัยส่วนมากมีข้อมูล อธิบายการคำนวณขนาดกลุ่มตัวอย่างที่ชัดเจน มีการประเมินผลการศึกษาโดยผู้ประเมินซึ่งไม่ทราบข้อมูลการ แบ่งกลุ่มที่ศึกษา และมีค่าคะแนน Jadad มากกว่าหรือเท่ากับ 3 คะแนน
สรุป: จากหลักฐานที่มีอยู่ในปัจจุบัน พบเทคนิคที่เหมาะสมที่สุดสำหรับการสกัดกั้นข่ายประสาทของแขนแต่ละบริเวณดังนี้ การฉีดยาชาภายในหรือ ภายนอกปลอกหุ้มข่ายประสาทของแขน, การฉีดยาชาหลายตำแหน่งภายในทุกกลุ่มของข่ายประสาท ทั้งกลุ่มหลัก และกลุ่มย่อยๆที่กระจายอยู่โดยรอบ, การฉีดยาชาเพียงตำแหน่งเดียวที่บริเวณหลังต่อหลอดเลือดแดง และการ ฉีดยาชา 2 ตำแหน่งบริเวณเส้นประสาท musculocutaneous ร่วมกับการฉีดยาชาล้อมรอบหลอดเลือดแดง สำหรับ การสกัดกั้นข่ายประสาทของแขนบริเวณระหว่างกล้ามเนื้อ scalene, บริเวณเหนือกระดูกไหปลาร้า, บริเวณล่าง ต่อกระดูกไหปลาร้าและบริเวณรักแร้ตามลำดับ สำหรับการเปรียบเทียบระหว่างบริเวณต่างๆ ในการเข้าหาข่าย ประสาทของแขน หากเลือกใช้เทคนิคที่เหมาะสมในการศึกษา พบว่า การเข้าหาข่ายประสาทของแขนบริเวณ เหนือกระดูกไหปลาร้า, บริเวณล่างต่อกระดูกไหปลาร้า และบริเวณรักแร้มีประสิทธิผลใกล้เคียงกัน อย่างไรก็ตาม การเข้าหาข่ายประสาทบริเวณล่างต่อกระดูกไหปลาร้าอาจมีประสิทธิภาพที่ดีกว่าเนื่องจากใช้การฉีดยาชาเพียง ตำแหน่งเดียวเท่านั้น ส่วนการวางสายสำหรับบริหารยาชาอย่างต่อเนื่อง พบว่า การวางสายบริเวณล่างต่อกระดูก ไหปลาร้าใช้เวลาในการทำหัตถการสั้นกว่าและให้ผลระงับปวดในช่วงวันแรกหลังการผ่าตัดดีกว่า เมื่อ เปรียบเทียบกับการวางสายบริเวณเหนือกระดูกไหปลาร้า อย่างไรก็ตามจากหลักฐานที่มีความน่าเชื่อถือที่มีอยู่ ในปัจจุบัน ยังคงพบข้อจำกัดในการระบุเทคนิคและบริเวณที่เหมาะสมที่สุดสำหรับการสกัดกั้นข่ายประสาท ของแขนโดยใช้เครื่องคลื่นเสียงความถี่สูงช่วยนำทาง จึงสนับสนุนให้มีการศึกษาในลักษณะของงานวิจัยเชิง ทดลองแบบสุ่มเพิ่มเติม เพื่อตอบคำถามที่ยังคงเป็นข้อสงสัย
