Abstract

The construction of SEED device was described in details. It was made of 10 ml, 20 ml disposable syringes and pediatric bacterial filter. The test set was made of 4.5 mm. endotracheal connectors, disposable medical glove and disposable 50 ml NSS bag (Baxter®) the clinical use of SEED device in anesthetic practice was also described both in children and adult.

 

วิธีการสร้าง การทดสอบและการใช้งานทางวิสัญญีวิทยาของอุปกรณ์ตรวจสอบการใส่ท่อ Endotracheal tube เข้าหลอดลมและหลอดอาหารแบบใหม่

บทนำ: วิธีการตรวจสอบการใส่ท่อเข้า trachea หรือหลอดอาหารมีได้หลายวิธี วิธีที่นิยมใช้กันมานาน และราคาไม่สูง คือ การใช้ Esophageal Detecting Device (EDD) แต่มักเป็นอุปกรณ์สำเร็จรูปที่เราต้องจัดซื้อ จากต่างประเทศ ซึ่งยังคงมีราคาค่อนข้างสูงอยู่

วัตถุประสงค์: ทดลอง ค้นคว้า และประดิษฐ์ EDD โดยใช้ชื่อ “Srinagarind Endotracheal & Esophageal Detecting device” (SEED device) ตามแนวพระราชดำริเศรษฐกิจ แบบพอเพียงจากวัสดุอุปกรณ์ในห้องผ่าตัด เพื่อลดราคาค่าใช้จ่ายของอุปกรณ์ลง

วิธีการ: SEED device มี ส่วนประกอบดังนี้ a) Disposable syringe 20 ml (TerumoR) 1 ชิ้น b) Disposable syringe 10 ml (TerumoR) 1 ชิ้น c) ส่วนปลายของก้านกระบอกฉีด10 ml เท่านั้น 1 ชิ้น (B / Braun R: PerifixR จาก epidural block set) และ d) Pediatric Bacterial Filter (Tyco / healthcareR) 1 ชิ้น ประกอบชิ้นส่วนต่าง ๆ ของ SEED device เข้า ด้วยกัน โดยการถอดชิ้นส่วนที่ใช้เป็นลูกลอยจากปลายก้านกระบอกฉีดยา c นำมาใส่ในตัวกระบอกฉีดยา b เพื่อใช้ดูการเคลื่อนที่ของมันในการตรวจสอบ และประดิษฐ์ชุดทดสอบการทำงาน ประกอบด้วยถุงพลาสติกใส บรรจุ 0.9 % sodium chloride ขนาด 50 มล. (BaxterR) ที่ใช้แล้ว 1 ถุง นิ้วถุงมือยางแบบใช้แล้วทิ้ง 1 ชิ้น และ Endotracheal tube connector # 4.5 มม. 2 ชิ้น ก่อนนำไปใช้งานจริง ต้องมีการทดสอบการทำงานของ SEED device ทุกครั้ง เพื่อทดสอบว่าใส่ท่อช่วยหายใจเข้าท่อเข้าหลอดลม หรือเข้าหลอดอาหาร (esophagus)

ผลการศึกษา: การทดสอบใช้งานทางคลินิกในผู้ใหญ่ หลังจากเราใส่ท่อ endotracheal tube และ blow cuff ให้ได้ cuff pressure 10 - 15 mm Hg แล้วนำSEED device มาสวมเข้าที่ endotracheal tube connector แล้วดึง ก้าน syringe 20 ml เข้าหาตัวโดยเร็วภายใน 0.5 - 1.0 วินาที ถ้าลูกลอยไม่เลื่อนไหลลงมา แสดงว่าท่ออยู่ใน หลอดลม แต่ถ้าลูกลอยเลื่อนไหลลงมาต่ำสุด แสดงว่าท่ออยู่ในหลอดอาหาร ในเด็ก (น้ำหนักตัว 10 - 20 กก.) ก่อนตรวจสอบให้ดันลูกลอยขึ้นไปเพียงครึ่งหนึ่งของ syringe 10 ml และดึงก้าน syringe 20 ml เข้าหาตัวผู้ทดสอบ ไม่เกิน 10 ml ภายใน 0.5 - 1.0 วินาที การแปลผลเหมือนผู้ใหญ่ หลังจากทดสอบการใช้งานแล้วพบว่า SEED สามารถใช้ตรวจสอบตำแหน่งของการใส่ท่อช่วยหายใจได้จริง

สรุป: SEED device เป็นเครื่องมือตรวจเชิงกล สำหรับยืนยันความถูกต้องในการใส่ท่อ endotracheal tube ให้แก่ผู้ป่วยทางด้านวิสัญญีวิทยา และเวชศาสตร์ ฉุกเฉินที่มีประโยชน์สูง ราคาถูก น้ำหนักเบาและพกพาสะดวก
