Mortality and morbidity in pediatric patients underwent MRI and CT under general anesthesia

Background: Recently the number of diagnostic and therapeutic MRI and CT in children has increased dramatically. Most of these procedures require sedation or anesthesia to achieve a certain degree of cooperation or immobilization necessary to successfully complete the procedures. Risk of anesthesia in the remote area, especially for children who have illness, may increase.
Objectives: The aim of this study was to identify mortality, morbidity and risk factors associated with morbidity and determine incidence of adverse events in children underwent MRI and/or CT under general anesthesia.
Methods: A retrospective descriptive study was performed at Ramathibodi Hospital using registry of patients who underwent CT and/or MRI under general anesthesia during January 2010 to December 2012. Anesthetic records and chart review was performed in all patients under 15 years of age. Demographic data and adverse events were recorded and analyzed.
Results: Seven hundred and twenty pediatric patients underwent MRI and/or CT under general anesthesia in the period. Two-hundred eighty patients were excluded because they were admitted in other hospital than Ramathibodi and the complete record review was not possible. Data of total 440 pediatric patients were analyzed. The mean age was 4.45+3.47 years old; ASA physical status was mostly II and III. Incidence of bradycardia, airway management, oxygen desaturation, delayed emergence and convulsion were 2.95%, 0.90%, 0.68%, 0.45%, 0.22%, respectively. There was no mortality.
Conclusion: Anesthesia for MRI and CT in pediatric patients is not without risk; with the overall 5.22 % morbidity. The risk factors were analyzed and discussed.
อัตราตายและพยาธิภาวะในผู้ป่วยเด็กได้รับการดมยาสลบเพื่อทำ MRI และ CT
บทนำ: ในการถ่ายภาพด้วยคลื่นแม่เหล็ก (MRI) และ เอกซเรย์คอมพิวเตอร์ (CT) ในผู้ป่วยเด็ก ต้องให้ผู้ป่วยนอนนิ่งนานพอที่จะทำได้สำเร็จ จึงมักต้องใช้บริการทางวิสัญญีเพื่อบริหารยาให้ผู้ป่วยสงบหรือดมยาสลบผู้ป่วย บริการทางวิสัญญีในภาวะดังกล่าวซึ่งอยู่นอกห้องผ่าตัด โดยเฉพาะอย่างยิ่งทำในเด็กที่เจ็บป่วย อาจมีความเสี่ยงมากกว่าการดมยาสลบระงับความรู้สึกธรรมดา
วัตถุประสงค์: เพื่อศึกษาอัตราตายและพยาธิภาวะในผู้ป่วยเด็กที่มาทำ MRI และ CT โดยใช้การดมยาสลบและ ปัจจัยเสี่ยงที่เกี่ยวกับอุบัติการณ์เหล่านั้น
วิธีการศึกษา: เป็นการศึกษาเชิงพรรณาย้อนหลัง โดยการทบทวนบันทึกการดมยาสลบในผู้ป่วยอายุ 15 ปีลงมา ที่มาทำ MRI และ CT ที่โรงพยาบาลรามาธิบดี ในช่วงเดือนมกราคม พ.ศ.2553 ถึง ธันวาคม พ.ศ.2555 บันทึกข้อมูลผู้ป่วย การตายภาวะผิดปกติต่างๆที่เกิดขึ้นระหว่างและหลังการดมยาสลบ
ผลการศึกษา: ผู้ป่วย 720 ราย ได้รับการดมยาสลบเพื่อทำ MRI และ/หรือ CT ในช่วงดังกล่าว แต่เป็นผู้ป่วยจากโรงพยาบาลอื่นนอกรามาธิบดี 280 ราย ซึ่งไม่สามารถเก็บข้อมูลหลังทำได้อย่างสมบูรณ์ ดังนั้นจึงวิเคราะห์ข้อมูลได้จากผู้ป่วย 440 ราย อายุเฉลี่ย 4.45 + 3.47 ปี, ASA physical status 2 หรือ 3 อุบัติการณ์ไม่พึงประสงค์ที่พบได้แก่ หัวใจเต้นช้า, ปัญหาเกี่ยวกับทางเดินหายใจ, ความเข้มข้นของออกซิเจนในเลือดต่ำ, ฟื้นช้ากว่าที่ควร และชัก เท่ากับ ร้อยละ 2.95, 0.90, 0.68, 0.45 และ 0.22 ตามลำดับ ไม่มีผู้ป่วยตาย
สรุป: การดมยาสลบเพื่อทำ MRI และ CT ในผู้ป่วยเด็กมีความเสี่ยง การศึกษานี้พบพยาธิภาวะร้อยละ 5.22 การศึกษานี้ได้วิเคราะห์ปัจจัยที่เกี่ยวข้อง
