Abstract

Background: Management of reusable supraglottic airway devices in a large hospital is a complex task. Limited supply is secondary to device losses and damages. We have set up the new management system and report this follow up results 

Objectives: To evaluate outcomes of the new supraglottic airway management after implementation of the new management scheme in aspect of 1) number of uses and rate of losses 2) rate of damaged devices and device maintenance and 3) cost analysis.

Methods: A single center, retrospective data analysis of usage statistic, rate of device loss, rate of damaged devices and repair. Preliminary cost analysis by institutional budget unit is also included.

Result: Following changes in management of supraglottic devices implemented in March 2012, we reported zero device loss and 44 damaged LMAs, all of which are LMA ProSeal. With proper repair and pre-use test, all damaged LMAs can be returned to function and gain wide acceptance from users. By 2012 year end, the LMA ProSeal no 3 and no 4 are the most frequently used LMA. The maximum use was LMA ProSeal no 4, at 90 times. The average use was 47.2 times for ProSeal no 3 and 54.7 times for ProSeal no 4. The cost analysis confirmed that the use of LMA ProSeal is cost effective when compared with disposable LMA-Supreme.

 

การควบคุมดูแลอุปกรณ์ช่วยหายใจเหนือกล่องเสียงชนิด LMA และ i-gel ในโรงพยาบาลขนาดใหญ่ ปี พ.ศ. 2555 การติดตามผลหลังดำเนินการ 1 ปี

บทนำ: การควบคุมดูแลอุปกรณ์ช่วยหายใจเหนือกล่องเสียงชนิดใช้ซ้ำได้ในโรงพยาบาลขนาดใหญ่เป็นงานที่ซับซ้อนและมีความท้าทายสูง ปัญหาที่พบบ่อยได้แก่ อุปกรณ์สูญหาย หรือ อุปกรณ์ชำรุด ซึ่งทำให้จำนวนอุปกรณ์ไม่พอใช้ ที่โรงพยาบาลศิริราชมีการเปลี่ยนแปลงระบบการควบคุมดูแลอุปกรณ์เหล่านี้ใหม่จึงมีการรายงานเพื่อติดตามผล

วัตถุประสงค์: เพื่อประเมินผลการดำเนินงานของระบบใหม่ในด้าน 1) จำนวนการใช้งานและอัตราการสูญหาย 2) อัตราการชำรุดและการซ่อมแซม 3) การประเมินค่าใช้จ่ายและความคุ้มทุน

วิธีการ: เป็นการศึกษาโดยการเก็บข้อมูลย้อนหลัง โดยประเมินตัวเลขสถิติอัตราการใช้งาน อัตราการหายและการชำรุดรวมถึงการประเมินต้นทุนและความคุ้มทุนเบื้องต้น

ผลการศึกษา: พบว่าไม่มีอุปกรณ์สูญหายหลังปรับเปลี่ยนวิธีควบคุมดูแลใหม่แต่มีรายงานอุปกรณ์ LMA-ProSeal ชำรุด 44 อัน ซึ่งสามารถซ่อมแซมให้ผ่านการตรวจสอบก่อนใช้จริงและได้รับการยอมรับจากผู้ใช้งาน สรุปสถิติในปีพ.ศ. 2555 พบว่า LMA ProSeal เบอร์ 3 และเบอร์ 4 ถูกใช้งานมากที่สุดโดยสถิติสูงสุดได้แก่ LMA-ProSeal เบอร์ 4 (90 ครั้ง) จำนวนการใช้งานโดยเฉลี่ยสำหรับ LMA-ProSeal เบอร์ 3 คือ 47.2 ครั้งและ 54.7 ครั้งสำหรับ LMA Proseal เบอร์ 4 ผลการประเมินต้นทุนเบื้องต้นสนับสนุนการใช้ LMA ProSeal มีความคุ้มค่าเมื่อเปรียบเทียบกับการใช้ LMA-Supreme ซึ่งเป็นอุปกรณ์ใช้ครั้งเดียว
