Non - ventilatory management in ARDS

Background: Acute respiratory distress syndrome (ARDS) is a syndrome that causes injury to the lung. It may be results from direct insult (e.g. lung infection) or indirect insult such as sepsis, transfusion - related acute lung injury, or postoperative ARDS. The mainstays of management in ARDS are including specific and supportive treatments. According to the previous literatures, not only specific treatment but also optimal supportive care can improve patient survival. We briefly described the evidences of supportive treatment in ARDS using non - ventilatory approaches, which composed of pharmacologic and non - pharmacologic adjuncts. We identified their mechanisms, advantages, and disadvantages.
การรักษา ARDS เเบบประคับประคองโดยไม่ใช้เครื่องช่วยหายใจ
บทนำ: ภาวะทางเดินหายใจล้มเหลวเฉียบพลัน หรือ acute respiratory distress syndrome (ARDS) เป็น กลุ่มอาการ ที่เกิดจากการมีพยาธิสภาพโดยตรงต่อเนื้อปอด เช่น ปอดอักเสบจากการติดเชื้อ หรือเกิดจากการ บาดเจ็บที่ระบบอื่น ส่งผลให้มีการอักเสบ และเกิดการบาดเจ็บที่ปอดตามมา เช่น การติดเชื้อในกระแสเลือด การได้รับส่วนประกอบของเลือด หรือการเกิด ARDS ภายหลังการผ่าตัด เป็นต้น การดูแลผู้ป่วย ARDS นอกจาก จะต้องรักษาที่สาเหตุแล้ว การรักษาแบบประคับประคองบางชนิด ยังสามารถลดอัตราตายของผู้ป่วยลงได้ บทความนี้จะกล่าวถึงการรักษา ARDS แบบประคับประคองโดยไม่ใช้เครื่องช่วยหายใจ ซึ่งประกอบด้วยการ ใช้ยา และไม่ใช้ยา โดยจะสรุปกลไกการทำงาน ข้อดี และข้อเสีย ของการรักษาแบบประคับประคองโดยไม่ใช้เครื่องช่วยหายใจด้วยวิธีต่างๆ
