The optimal concentration of fentanyl in combination with 0.0625% Bupivacaine for Patient - controlled epidural analgesia after lower abdominal surgery

Background: Patient - controlled epidural analgesia with combination of local anesthetic and opioid is the most effective method of providing pain relief for abdominal surgery. Fentanyl is popular used but the optimal concentration remains unclear.
Objective: To determine three commonly used concentrations of fentanyl in combination with bupivacaine that provides optimal analgesia and minimal adverse effects in patients undergoing lower abdominal surgery.
Methods: This study was a prospective, triple - blind, randomized controlled trial of 36 patients undergoing lower abdominal gynecologic surgery. Patients were randomly allocated to 3 groups to receive fentanyl 1 mcg/mL (F1), 2 mcg/mL (F2) and 3 mcg/mL (F3) in combination with bupivacaine 0.0625%. PCEA was setting as followed: initial loading volume 15 mL, background infusion 5 mL/h, bolus volume 3 mL and lockout interval 10 min. Pain at rest, pain on coughing, total analgesics consumption, total infusion volume and frequencies of analgesic drug delivery at 1, 2, 4, 8, 18 and 24 h were recorded. At the same time, nausea, sedation, and pruritus were also assessed.
Results: There were 2 patients in group F1 were not eligible for analysis. Of 34 patients who remained in the study, there was no significant difference in baseline characteristics between groups. Total analgesics consumption and unsatisfactory pain was significantly higher in group F1 (P < 0.05). Patients who had sedation scores > 1 were significantly higher in group F3 (P < 0.05).
Conclusion: Fentanyl 2 mcg/ml combined with 0.0625% bupivacaine for PCEA was the optimal concentration that provided excellent analgesia and few side effects after lower abdominal surgery.

วิธีการศึกษา: เป็นการศึกษา แบบ prospective triple-blind randomized controlled trial มีผู้เข้าร่วมการศึกษาทั้งหมด 36 คน เป็นผู้ป่วยนรีเวช ที่ได้รับการวินิจฉัยว่าเป็นเนื้องอกมดลูกชนิดไม่ร้ายแรง (myoma uteri) และมารับการผ่าตัดมดลูกทางหน้าท้อง (transabdominal hysterectomy) โดยวิธีการระงับความรู้สึกแบบทั่วตัวร่วมกับการให้ยาระงับปวดทางช่องเหนือ ไขสันหลัง (epidural analgesia) แบ่งเป็น 3 กลุ่มตามขนาดยา Fentanyl ที่ใช้ผสมร่วมกับยาชาเฉพาะที่ 0.0625% Bupivacaine คือ Fentanyl 1 มคก./มล. (F1), 2 มคก./มล. (F2) และ 3 มคก./มล. (F3) ตามลำดับ เริ่มต้นให้ 15 มล. แล้วให้หยดต่อเนื่องในอัตรา 5 มล./ชม. จนเสร็จผ่าตัด จากนั้นให้ต่อโดยเทคนิค PCEA ตั้งโปรแกรมให้ หยดต่อเนื่องในอัตราพื้นฐาน 5 มล./ชม. ผู้ป่วยสามารถกระตุ้นเครื่องให้จ่ายยาเพิ่มได้ครั้งละ 3 มล. ต่อช่วงเวลา ทุก 10 นาที บันทึกปริมาณยาแก้ปวดที่ได้รับ จำนวนครั้งที่ได้รับยา คะแนนประเมินความปวด (VNRS scores) ที่ 1, 2, 4, 8, 18 และ 24 ชั่วโมงหลังผ่าตัด รวมถึงผลข้างเคียงไม่พึงประสงค์
ผลการศึกษา: ในจำนวนทั้งหมด 36 ราย มี 2 รายจากกลุ่ม F1 ที่ออกจากการศึกษา ลักษณะประชากรทั้งสามกลุ่มไม่มีความแตกต่างกันอย่างมีนัยสำคัญ ทางสถิติ ปริมาณการใช้ยาแก้ปวดและจำนวนครั้งของการได้ยาแก้ปวดของกลุ่ม F1 มากกว่ากลุ่ม F2 และ F3 อย่างมีนัยสำคัญทางสถิติ กลุ่ม F1 มีจำนวนผู้ป่วยที่มีการระงับปวดที่ไม่ดี (unsatisfactory pain, VNRS score ////////// 3) ทั้งความปวดขณะพักและขณะเคลื่อนไหว มากกว่ากลุ่ม F2 และ F3 อย่างมีนัยสำคัญทางสถิติ ขณะที่กลุ่ม F3 พบภาวะง่วงซึมมากกว่ากลุ่ม F1 และ F2 อย่างมี นัยสำคัญทางสถิติ
สรุป: การใช้ Fentanyl ขนาด 2 มคก./มล. ผสมร่วมกับยาชาเฉพาะที่ 0.0625% Bupivacaine โดยวิธี PCEA สำหรับการผ่าตัดช่องท้องส่วนล่าง มี ประสิทธิภาพในการระงับปวดที่ดีเพียงพอขณะที่ผลข้างเคียงไม่พึงประสงค์ตํ่า
