Guidelines for the management of blood and blood component with massive transfusion protocol at Srinagarind Hospital

Abstract

The large volume of blood and blood components transfusion (massive transfusion), the treatment of choice, can rescue patients with severe and acute hemorrhage, which is a major cause of cardiac arrest in the operating room or delivery room. This system requires coordination between the multidisciplinary team (include the anesthesiologist, the surgeon, the operating room and labor room personnel, unit of accident – emergency, laboratory diagnosis unit and the Central Blood Bank), and systems management arrangement with a union. It is the policy of the tertiary hospitals to develop and practices in the preparation and support for the occurrence of severe bleeding, including the establishment of a central coordination point in each scene.

Multidisciplinary group of Srinagarind Hospital, Faculty of Medicine, Khon Kaen University have jointly drafted a preliminary guideline on the management of massive using the blood and blood components transfusion (massive transfusion protocol: MTP) and applied it in the context of each ward or department. This preliminary and prototype guide start from find out a risk factor for both search and surveillance of patients at risk of bleeding from accidentalemergency ward and obstetrics ward. Teams meeting prepare case scenario at both targeted wards, summarize the roles and responsibilities to be inconsistencies in practice, including coordination with the central blood bank to the availability of blood and blood components according to massive transfusion protocol. The properly and efficiently treatment base on medical information from the immediate and accurately results of laboratory tests, are considered to result in rapid improvement. Main of this project is patient safety and reduces costs associated with hospitalization.

การใช้แนวทางการบริหารจัดการภาวะการให้เลือดและส่วนประกอบ ของเลือดปริมาณมาก (massive transfusion protocol: MTP) โรงพยาบาลศรีนครินทร์ คณะแพทยศาสตร์ มหาวิทยาลัยขอนแก่น

การให้เลือดและส่วนประกอบของเลือดทดแทนปริมาณมาก (massive transfusion) เพื่อช่วยกู้ชีพ ผู้ป่วยที่มีภาวะตกเลือดอย่างรุนแรงและเฉียบพลัน ซึ่งเป็นสาเหตุสำคัญของภาวะหัวใจหยุดเต้นในห้องผ่าตัด หรือห้องคลอด ต้องอาศัยการประสานงานระหว่างทีมสหสาขาวิชาชีพ และมีระบบการบริหารจัดที่ดีอย่างมี เอกภาพ ระหว่างวิสัญญีแพทย์, ศัลยแพทย์, ทีมบุคลากรในห้องผ่าตัด, ห้องคลอด, แผนกอุบัติเหตุ - ฉุกเฉิน ห้องปฏิบัติการชันสูตร และคลังเลือดกลาง ดังนั้นจึงถือเป็นนโยบายหลักของโรงพยาบาลระดับตติยภูมิ ในการเตรียมแนวทางปฏิบัติหรือระบบรองรับการเกิดภาวะตกเลือดที่รุนแรง รวมทั้งการจัดตั้งผู้ประสานงาน กลางในแต่ละจุดเกิดเหตุ

กลุ่มของสหสาขาวิชาชีพภายในโรงพยาบาลศรีนครินทร์ คณะแพทยศาสตร์ มหาวิทยาลัยขอนแก่น จึงได้ ร่วมกันร่างรูปแบบแนวทางต้นแบบในการบริหารจัดการภาวะการให้เลือดและส่วนประกอบของเลือดปริมาณมาก (massive transfusion protocol: MTP) และนำมาปรับใช้จริงตามบริบทของแต่ละหน่วยงาน โครงการสร้าง แนวทางต้นแบบฯ เริ่มจากการค้นหาผู้ป่วยกลุ่มเสี่ยงของภาวะเลือดออกโดยมีการเลือกปัจจัยเสี่ยงเพื่อค้นหา และเฝ้าระวังทั้งผู้ป่วยที่มีความเสี่ยงทางศัลยกรรมและทางสูติกรรม มีการประชุมเพื่อจัดเตรียมความพร้อม ณ จุดเกิดเหตุทั้งในตึกผู้ป่วยแผนกอุบัติเหตุ - ฉุกเฉิน, ห้องผ่าตัด และห้องคลอด เพื่อสรุปบทบาทและหน้าที่ให้ มีความสอดคล้องกันในแนวทางการปฏิบัติ รวมถึงการประสานงานกับคลังเลือดให้มีความพร้อมใช้ทั้งเลือด และส่วนประกอบของเลือดในรูปแบบ massive transfusion protocol และมีผลการตรวจทางห้องปฏิบัติการ ที่รายงานผลได้อย่างเร่งด่วน เพื่อเป็นข้อมูลให้แพทย์ได้ตัดสินใจเลือกวิธีการรักษาได้อย่างถูกต้อง แม่นยำ และมีประสิทธิภาพ โดยมีการพิจารณาถึงผลลัพธ์ในการรักษาที่รวดเร็ว ปลอดภัยและลดค่าใช้จ่ายของโรงพยาบาลร่วมด้วย
