การพัฒนาหุ่นจำลองเต้านม FON CMU เพื่อการสอนการเลี้ยงบุตรด้วยนมมารดา

Abstract

บทคัดย่อ
การพัฒนาหุ่นจำลองเพื่อใช้ในการเรียนการสอนด้านสุขภาพ มีส่วนสำคัญที่ช่วยให้ผู้เรียนเข้าใจและมีทักษะในการปฏิบัติงานก่อนปฏิบัติกับผู้รับบริการในคลินิก วัตถุประสงค์ของการวิจัยและพัฒนา (research anddevelopment) ครั้งนี้ เพื่อพัฒนาหุ่นจำลองเต้านมใช้ในการเรียนการสอนเกี่ยวกับการเลี้ยงบุตรด้วยนมมารดาในหลังคลอด ซึ่งมีการดำเนินงาน 3 ขั้นตอนดังนี้ 1) การเตรียมการเพื่อพัฒนาหุ่นจำลอง 2) การพัฒนาหุ่นจำลอง และ3) การนำหุ่นจำลองไปทดลองใช้และประเมินคุณภาพ กลุ่มตัวอย่าง คือ นักศึกษาพยาบาลและพยาบาลที่ปฏิบัติงานในหน่วยหลังคลอดในฐานะผู้สอนและให้คำแนะนำมารดาหลังคลอด ในขั้นตอนที่ 1 จำนวน 20 คน ขั้นตอนที่2 จำนวน 57 คน และ ขั้นตอนที่ 3 จำนวน 57 คน เครื่องมือที่ใช้ได้แก่ แนวคำถามเกี่ยวกับหุ่นจำลองเต้านม และแบบประเมินคุณภาพหุ่นจำลองเต้านม ซึ่งได้รับการตรวจสอบจากผู้ทรงคุณวุฒิ 3 ท่าน ได้ค่าดัชนีความตรงตามเนื้อหา (CVI) เท่ากับ 0.96 และหาค่าความเชื่อมั่นของแบบประเมินคุณภาพหุ่นจำลองเต้านมโดยการทดสอบซํ้าได้ค่าความเชื่อมั่นเท่ากับ 0.92 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย การทดสอบ Friedman และ WilcoxonSigned Ranks Test
ผลการวิจัย ได้หุ่นจำลองเต้านม FON CMU ซึ่งทำด้วยซิลิโคนมีโครงสร้างเป็นพลาสติก ที่สามารถใช้อธิบายโครงสร้างและลักษณะทั้งภายนอกและภายในของเต้านมมารดาหลังคลอด รวมทั้งฝึกทักษะการเตรียมและให้การดูแลเต้านมมารดาหลังคลอด (การนวด การประคบ และการบีบนํ้านม)ได้ เมื่อนำมาทดลองใช้และประเมินคุณภาพมีค่าเฉลี่ยคะแนนคุณภาพโดยรวมอยู่ในระดับมากที่สุด (x_= 4.72, S.D. = 0.23) และค่าเฉลี่ยคะแนนคุณภาพในแต่ละคุณสมบัติอยู่ในระดับมากที่สุดทุกคุณสมบัติ (x_= 4.53-4.96, S.D = 0.23-0.83) เมื่อเปรียบเทียบคะแนนคุณภาพหุ่นจำลองเต้านม FON CMU ที่พัฒนาขึ้นใหม่กับหุ่นจำลองเต้านมที่มีอยู่เดิม พบว่า หุ่นจำลองเต้านม FONCMU มีคะแนนคุณภาพดีกว่าหุ่นจำลองเต้านมครึ่งตัว (γ2= 6.57, p<0.001) และหุ่นจำลองเต้านมที่ทำด้วยผ้า( γ2= 6.57, p<0.001) โดยสรุปหุ่นจำลองเต้านม FON CMU ที่ทำด้วยซิลิโคนนี้สามารถไปใช้เป็นสื่อการเรียนรู้ใหม่ที่ทำให้เพิ่มความเข้าใจและสามารถฝึกทักษะในการเตรียมและให้การดูแลเต้านมมารดาหลังคลอดได้ดียิ่งขึ้น

Abstract
Using a model for health education can help learners to better understand and enhancetheir practice skills before engaging in clinical practice with clients. The objective of this researchand developmental study was to develop a ‘FON CMU breast model’ to use for breastfeedinghealth education with postpartum mothers. The process consisted of the following three steps:1) preparing for model development; 2) model development; and 3) trial and evaluating quality.The sample included 107 nursing students and staff nurses who conducted breastfeeding educationfor postpartum mothers; 20 persons in step 1, 30 in step 2, and 57 in step 3. The researchinstruments consisted of an interview guide and a breast model quality evaluation form, whichwere examined by experts for content validity. The content validity index of breast model qualityevaluation form was 0.96 and the test-retest reliability was 0.92. Data were analyzed usingdescriptive statistics, Friedman test, and Wincoxon Signed Rank Test.
The ‘FON CMU breast model’ was developed using silicone with an outer plastic structure. Thismodel was helpful in explaining anatomy and physiology of the breast and in demonstrating breastpreparation techniques and care (breast massage, warm compression, and breast milk expression).After trial and an evaluation of the quality of the “FON CMU breast model”, the overall mean scorewas at the highest level (x_= 4.72, S.D. = 0.23) as well as for the different properties of the model(x_= 4.53-4.96, S.D. = 0.23-0.83) When comparing the “FON CMU breast model” with a half bodyand cotton breast models, the quality score of ‘FON CMU breast model’ was higher than half bodybreast model (γ2 = 6.57, p< 0.001) and cotton breast model (γ2 = 6.57 p<0.001). To sum up, thisFON CMU breast model could use as a new teaching media to better understand and practice onpreparation and care of breast for postpartum mothers.
