การพัฒนาแนวปฏิบัติในการฝึกการสื่อสารบุคคลออทิสติกโดยใช้รูปภาพ

Abstract

บทคัดย่อ
การศึกษาเชิงพัฒนาครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาแนวปฏิบัติในการฝึกการสื่อสารบุคคลออทิสติกโดยใช้รูปภาพ กลุ่มตัวอย่าง ได้แก่ 1) บุคลากรทางสุขภาพที่พัฒนาแนวปฏิบัติทางคลินิก จำนวน 3 คน 2) บุคลากรทางสุขภาพที่ทดลองใช้แนวปฏิบัติเพื่อดูความเป็นไปได้ของแนวปฏิบัติทางคลินิกที่สร้างขึ้น จำนวน 9 คน และ3) บุคคลออทิสติก ที่เข้ารับการรักษาในหอผู้ป่วยจิตเวชเด็กโรงพยาบาลมหาราชนครเชียงใหม่ ระหว่าง มกราคมพ.ศ. 2555 ถึง สิงหาคม พ.ศ. 2555 จำนวน 7 คน ที่มีอายุระหว่าง 7-22 ปี
ผลการวิจัย พบว่า
การพัฒนาแนวปฏิบัติในการฝึกการสื่อสารบุคคลออทิสติกโดยใช้รูปภาพ ประกอบด้วยสาระสำคัญ ดังนี้1) การพิทักษ์สิทธิผู้ป่วยและจริยธรรม 2) การประเมินความสามารถทางการสื่อสารของบุคคลออทิสติก 3) การฝึกการสื่อสารโดยใช้รูปภาพของบุคคลออทิสติก 4) การให้ความรู้เกี่ยวกับการฝึกการสื่อสารโดยใช้รูปภาพของบุคคลออทิสติก 5) การฝึกต่อเนื่องในการฝึกการสื่อสารโดยใช้รูปภาพของบุคคลออทิสติก 6) การพัฒนาคุณภาพบริการ หลังจากการทดลองใช้แนวปฏิบัติ ความคิดเห็นของบุคลากรทั้ง 9 คน มีดังนี้ แนวปฏิบัติมีความชัดเจนสามารถนำไปปฏิบัติได้และมีประโยชน์ต่อหน่วยงานเข้าใจได้ง่าย บุคลากรมีความพึงพอใจในการใช้แนวปฏิบัติทางคลินิกอยู่ในระดับมาก ความพงึ พอใจของผ้ปู กครองบุคคลออทสิ ติกทัง้ หมดมีความพึงพอใจในระดับมากและบุคคลออทิสติกทั้งหมดมีความสามารถทางการสื่อสาร โดยผ่านตามเกณฑ์การประเมินการฝึกการใช้ภาพเพื่อแลกเปลี่ยนสื่อสารขั้นที่ 1 เพิ่มขึ้นจำนวน 6 ใน 7 รายคิดเป็น ร้อยละ 85.71
ผลการศึกษาครั้งนี้ พบว่าแนวปฏิบัติในการฝึกการสื่อสารบุคคลออทิสติกโดยใช้รูปภาพ ที่พัฒนาขึ้นอย่างเป็นระบบตามหลักฐานความรู้เชิงประจักษ์ สามารถนำไปใช้ในการปฏิบัติงานได้จริง ผู้ศึกษาเห็นควรให้มีการนำแนวปฏิบัติในการฝึกการสื่อสารบุคคลออทิสติกโดยใช้รูปภาพ ไปสู่ขั้นตอนการนำไปใช้เพื่อประเมินประสิทธิผลและเผยแพร่ต่อไป

Abstract
Development of clinical practice guidelines (CPGs) for communication practice people withautism use PECS will lead to enhanced nursing in the same direction. As a result, thecommunication practice autism people with PECS had appropriate, effective and up to date.This developmental study aimed to develop clinical practice guidelines for children with autismin the Child Psychiatric Ward, Maharaj Nakorn Chiang Mai hospital based on the developmentof clinical practice guideline framework of the National Health and Medical Research Council(NHMRC,1998). The study subject included 1) 3 health care personnel who developed CPGsand 9 health care personnel who implemented CPGs for feasibility testing of the guidelines and2) 7 people which autism aged between 7-22 years who were admitted to the Child PsychiatricWard, Maharaj Nakorn Chiang Mai hospital between January 2012 and August 2012.The resultsof this study revealed that guidelines for people with autism PECS in the Child Psychiatric Ward,Maharaj Nakorn Chiang Mai hospital composed of 1) patients- rights and ethics, 2) to evaluatethe communication of autistic people, 3) practice of communication with PECS of autistic people,4) education for autistic people and caregiver, 5) continuing of care and 6) monitoring and qualityimprovement.
After applying the clinical practice guidelines, all health care team reported that the CPGswere feasible to practice, easily to understand, uncomplicated, useful and convenient to apply.
The health care team who used the CPGs were satisfied with the implementation. Accordingto clinical outcome, it was found that all of parents of people with autism have satisfaction havea high level of satisfaction and the communication ability of people with autism found that allseven subjects passed the criteria practice of PECS phase 1 further 6 out of 7 were 85.71percent.
This study found that the clinical practice guidelines based on the evidence could besystematically developed and applied in the unit. Researcher suggests that the clinical practiceguidelines should be further implemented to test for its efficiency and then be disseminated.
