The Effect of Promoting a Nutritional Self-Management Program on Persons Receiving Hemodialysis

Abstract

            End Stage Renal Disease (ESRD) is the last stage of chronic kidney disease. Many people live long lives while having hemodialysis. However, many of these patients have malnutrition. Therefore, they should be encouraged to manage their nutritional status on their own. This one-group, before-and-after study design aimed to evaluate the effect of promoting nutritional self-management on the nutritional status of persons undergoing hemodialysis. A purposive sample consisted of 26 participants with ESRD receiving hemodialysis treatment at a community hospital in Chonburi province. All participants were promoted to learn and manage their nutritional status within 12 weeks, under a self-management plan developed by the researcher. The instruments for data collection consisted of a demographic data record form and the Nutrition Status Record Form. Data were analyzed using descriptive statistics and the Wilcoxon-signed rank test.

          The results revealed that:
           1. After participating in the self-management promotion program, the subjects had statistically significantly higher median serum albumin levels than before participating in the program (p < .05).
            2. After participating in the self-management promotion program, the median weight of the subjects was not different from before participating in the program.
            3. After participating in the self-management promotion program, the subjects had a median body mass index that was not different from before participating in the program.
               The findings of this study demonstrate that promoting nutritional status under a self-management program can improve serum albumin in persons receiving hemodialysis. The intervention cannot improve body weight or body mass index. A new study design, such as a randomized control trial or a two-group, before-and-after design with a larger sample size is recommended for further study.
