Effects of an Enteral Feeding Nursing Care Bundle on Selected Clinical Outcomes in Critically Ill Medical Patients

Abstract

Enteral feeding in critically ill patients is crucial for promoting nutritional status. The objective of this study was to evaluate the effects of an enteral feeding nursing care bundle on selected clinical outcomes in critically ill medical patients who are intubated. The outcomes included incidence of aspiration during enteral feeding, comparisons of differences in mean gastric residual volume, number of meals received, number of bowel sounds, and time spent on tube feeding. The study divided participants into an experimental group (n = 49) receiving the enteral feeding nursing care bundle and a control group (n = 57) receiving standard hospital care over a 3-day period at a standard – level referral hospital. The research instruments included a data collection form for the nursing care bundle and the enteral feeding nursing care bundle. The content validity indexes were 0.83 and 0.88, respectively, and the item-objective congruence index was 0.80. Clinical outcomes were compared between the groups using generalized estimating equations, generalized linear models, and Cox proportional hazard regression for the incidence of aspiration, with a 95% confidence interval (CI).

The experimental group was 0.19 times less likely to aspirate compared to the control group (HR = 0.19, 95%CI: 0.07 - 0.48). The experimental group also had significantly higher means of gastric residual volume, bowel movement sounds, number of meals received, and time spent on tube feeding (p < .05).

The results reveal that the enteral feeding nursing care bundle helps reduce the risk of feeding complication. Therefore, nurses should be encouraged to utilize the nursing care bundle to reduce the risk of aspiration and promote the highest efficiency for enteral feeding in critically ill medical patients.
