ประสิทธิผลของการใช้แนวปฏิบัติที่เป็นเลิศสำหรับการส่งเสริมการเลี้ยงลูกด้วยนมแม่ โรงพยาบาลพบพระ จังหวัดตาก

บทคัดย่อ

การเลี้ยงลูกด้วยนมแม่ มีประโยชน์อย่างมากทั้งต่อแม่และลูก องค์การอนามัยโลก จึงแนะนำให้เลี้ยง ลูกด้วยนมแม่อย่างเดียว 6 เดือน ในประเทศไทยถึงแม้จะมีการรณรงค์การเลี้ยงลูกด้วยนมแม่อย่างต่อเนื่อง แต่อัตราการเลี้ยงลูกด้วยนมแม่อย่างเดียวที่ตํ่า ยังเป็นปัญหาที่สำคัญ การศึกษาครั้งนี้เป็นการศึกษา เชิงเปรียบเทียบชนิดศึกษาแบบย้อนหลังและไปข้างหน้าแบบไม่ควบคุมก่อนและหลังการใช้แนวปฏิบัติ ทางคลินิก โดยมีวัตถุประสงค์เพื่อศึกษาประสิทธิผลของการใช้แนวปฏิบัติที่เป็นเลิศสำหรับการส่งเสริม การเลี้ยงลูกด้วยนมแม่ ในหญิงที่ได้รับการดูแลในระยะตั้งครรภ์ ระยะคลอด และระยะหลังคลอดโรงพยาบาล พบพระ จังหวัดตาก กลุ่มตัวอย่างที่ใช้ในการศึกษาประกอบด้วยกลุ่มที่ได้รับการดูแลตามปกติ ระหว่างเดือน พฤษภาคมถึงเดือนสิงหาคม พ.ศ. 2552 จำนวน 12 คน และเป็นกลุ่มที่ได้รับการดูแลโดยใช้แนวปฏิบัติที่เป็นเลิศ ระหว่างเดือนพฤษภาคมถึงเดือนกรกฎาคม พ.ศ. 2553 จำนวน 11 คน เครื่องมือที่ใช้ในการดำเนินการ ศึกษา คือ แนวปฏิบัติที่เป็นเลิศสำหรับการส่งเสริมการเลี้ยงลูกด้วยนมแม่ของศูนย์อนามัยที่10 เชียงใหม่ ปี 2551 ดำเนินการศึกษาโดยใช้กรอบแนวคิดการใช้แนวปฏิบัติทางคลินิกของสมาคมพยาบาลออนทาริโอ (RNAO, 2002) เครื่องมือที่ใช้ในการรวบรวมข้อมูล คือ แบบบันทึกการเลี้ยงลูกด้วยนมแม่อย่างเดียว 6 สัปดาห์ หลังคลอด วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติทดสอบความน่าจะเป็นของฟิชเชอร์

ผลการวิจัยพบว่า

กลุ่มหญิงที่ได้รับการดูแลในระยะตั้งครรภ์ ระยะคลอด และระยะหลังคลอดโรงพยาบาลพบพระ ที่ได้ รับการดูแลโดยใช้แนวปฏิบัติที่เป็นเลิศมีจำนวนการเลี้ยงลูกด้วยนมแม่อย่างเดียว ในช่วง 6 สัปดาห์หลังคลอด มากกว่ากลุ่มที่ได้รับการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01 ผลการศึกษาครั้งนี้ยืนยันได้ว่าการ ใช้แนวปฏิบัติที่เป็นเลิศสำหรับการส่งเสริมการเลี้ยงลูกด้วยนมแม่ สามารถเพิ่มอัตราการเลี้ยงลูกด้วยนมแม่ได้

Abstract

Breastfeeding provides optimal benefits for mothers and infants. The World Health Organization (WHO) recommends mothers to exclusively breastfeed their infants during the first six months. However, currently low rate of exclusive breastfeeding is still a significant problem in Thailand. This comparative, retrospective and prospective uncontrolled before and after intervention study was conducted to determine the effectiveness of implementing best practice guidelines (BPGs) for breastfeeding promotion in women with antepartum care, intrapartum care and postpartum care, Phobphra hospital, Tak province. The study samples consisted of 12 pregnant women who received usual nursing care between May and August 2009 and 11 pregnant women who received care based on the BPGs between May and July 2010. The instrument was the BPGs for breastfeeding promotion developed by the Health Promotion Center 10, Chiang Mai (2008). The implementation of the CPGs model of Registered Nurse Association of Ontario (2002) was used as a framework for this study. Data collection tool consisted of exclusive breastfeeding 6 weeks postpartum record form. Data were analyzed using descriptive statistics and the Fisher’s exact probability test.

The results of study

Revealed that the women with antepartum care, intrapartum care and postpartum care, at Phobphra hospital in the BPGs implementing group had significantly higher rate of exclusive breastfeeding 6 weeks postpartum than the group with usual care (p < .01). The results confirm that implementation of BPGs for promoting exclusive breastfeeding can lead to improved quality outcome.
