นมมารดาเป็นอาหารที่ดีที่สุดสำหรับทารก แต่มารดาวัยรุ่นมีอัตราการเลี้ยงบุตรด้วยนม มารดาอย่างเดียวอยู่ในระดับตํ่า การส่งเสริมการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวในระยะ 6 เดือน มีปัจจัยหลายประการที่มีส่วนเกี่ยวข้อง การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาความ สัมพันธ์ของปัจจัยที่เกี่ยวข้องกับระยะเวลาในการเลี้ยงบุตรด้วยนมมารดาของมารดาวัยรุ่น ระหว่างเดือน พฤษภาคม ถึงเดือนสิงหาคม 2551 กลุ่มตัวอย่างคือ มารดาวัยรุ่นหลังคลอดปกติ และมี บุตรอายุตั้งแต่ 6 เดือน ถึง 1 ปี จำนวน 220 คน จากโรงพยาบาลชุมพรเขตอุดมศักดิ์ โรงพยาบาลสุราษฎร์ธานี และโรงพยาบาลระนอง คัดเลือกตัวอย่างแบบการสุ่มแบบหลาย ขั้นตอน เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถาม ความรู้ประโยชน์ของนมมารดาและวิธีการเลี้ยงบุตรด้วยนมมารดา แบบสอบถามเจตคติใน การเลี้ยงบุตรด้วยนมมารดาอย่างเดียว แบบสอบถามการได้รับบริการสาธารณสุข แบบสอบถาม ทักษะในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียว แบบสอบถามการได้รับคำแนะนำ /สนับสนุนจากบุคคลอื่น ผ่านการตรวจสอบความตรงตามเนื้อหา ดัชนีความตรงตามเนื้อหา มีค่าเท่ากับ 0.94, 0.85, 0.89, 0.87 และ 0.90 ตามลำดับ และทดสอบความเชื่อมั่นมี ค่าเท่ากับ 0.90, 0.91, 0.86, 0.83 และ 0.82 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติสหสัมพันธ์เพียร์สัน และสหสัมพันธ์ สเปียร์แมน

ผลการวิจัย พบว่า

ความรู้เกี่ยวกับประโยชน์ของนมมารดาและวิธีการเลี้ยงบุตรด้วยนมมารดา เจตคติในการเลี้ยงบุตรด้วย นมมารดาอย่างเดียว การได้รับบริการสาธารณสุข ทักษะในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียว และการได้ รับคำแนะนำ/สนับสนุนจากบุคคลอื่น มีความสัมพันธ์ทางบวกกับระยะเวลาใน การเลี้ยงบุตรด้วยนมมารดาของมารดาวัยรุ่น อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.01 (r = 0.835, 0.850, 0.350, 0.662 และ 0.851 ตามลำดับ) (p < 0.01)

ผลการวิจัยครั้งนี้สามารถนำผลของปัจจัยที่เกี่ยวข้องกับระยะเวลาในการเลี้ยงบุตรด้วยนมมารดามา พัฒนางานการพยาบาลและปรับปรุงระบบบริการให้มีประสิทธิภาพและประสิทธิผลมากยิ่งขึ้น โดยการสร้าง โปรแกรมส่งเสริมการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวของมารดาวัยรุ่นให้มีระยะเวลาที่ยาวนานยิ่งขึ้น

Abstract

Breast milk is the best food for an infant, but in adolescent mothers the exclusive breastfeeding rate is low. The promotion of exclusive breastfeeding for 6 months is related to several significant factors. The purpose of this research was to examine the relationship between factors related to exclusive breastfeeding duration among adolescent mothers during May to August, 2008. The study sample included 220 mothers with full term normal delivery and their infants aged between 6 months – 1 year old from Chumphorn Keat Udomsak hospital, Suratthani hospital and Ranong hospital, who were selected through multi-stage random sampling method. Data collection tools included the Personal Data from the Knowledge and Method of Exclusive Breastfeeding Scale, the Attitude Toward Exclusive Breastfeeding Scale, the Provision of Exclusive Breastfeeding Service Scale, the Exclusive Breastfeeding Skills and Personal Support Scale. All of the scales were approved for content validity, the content validity index were 0.94, 0.85, 0.89, 0.87 and 0.90 respectively and reliability were 0.90, 0.91, 0.86, 0.83 and 0.82 respectively. Data were analyzed using descriptive statistics, Pearson’s product moment coefficient correlation, and Spearman rank correlation.

The results of study

The knowledge and method of exclusive breastfeeding, attitude toward exclusive breastfeeding, provision of exclusive breastfeeding service, exclusive breastfeeding skills and personal support were statistically significantly positively correlated to duration of breastfeeding in adolescent mothers (r = 0.835, 0.850, 0.350, 0.662 and 0.851 respectively p < 0.01).

The results of this study could be used to develop a nursing intervention and improve the nursing service system to be more efficient and produce the exclusive breastfeeding program among adolescent mothers, in order to promote longer exclusive breastfeeding.
