การปฏิบัติการดูแลทารกเกิดก่อนกำหนดแบบแกงการูของพยาบาลและปัจจัยที่เกี่ยวข้อง

บทคัดย่อ

การดูแลทารกเกิดก่อนกำหนดแบบแกงการูก่อให้เกิดผลดีทั้งมารดาและทารก พยาบาลเป็นผู้ที่มีบทบาท สำคัญในการปฏิบัติการดูแลทารกเกิดก่อนกำหนดแบบแกงการู การวิจัยแบบสำรวจเชิงพรรณนาหาความสัมพันธ์ ครั้งนี้จึงมีวัตถุประสงค์ เพื่อศึกษาการปฏิบัติการดูแลทารกเกิดก่อนกำหนดแบบแกงการูของพยาบาลและ ความสัมพันธ์กับปัจจัยที่เกี่ยวข้อง ประชากรคือพยาบาล วิชาชีพจำนวน 161 คน ที่ปฏิบัติงานดูแลทารกเกิด ก่อนกำหนดในหออภิบาลทารกแรกเกิด หรือหอผู้ป่วยทารกแรกเกิดความเสี่ยงสูง ในโรงพยาบาลระดับตติยภูมิ ในภาคเหนือ จำนวน 7 แห่ง รวบรวมข้อมูลระหว่างเดือนธันวาคม 2552 ถึงเดือนมิถุนายน 2553 เครื่องมือ ที่ใช้ในการวิจัยประกอบด้วย 1) แบบสอบถามการปฏิบัติการดูแลทารกเกิดก่อนกำหนดแบบแกงการู 2) แบบสอบถาม ความรู้เกี่ยวกับการดูแลทารกเกิดก่อนกำหนดแบบแกงการู และ 3) แบบสอบถามปัจจัยที่เกี่ยวข้องกับการปฏิบัติ การดูแลทารกเกิดก่อนกำหนดแบบแกงการู วิเคราะห์ข้อมูลโดยสถิติพรรณนา สถิติสหสัมพันธ์พอยท์ไบซีเรียล และสถิติสหสัมพันธ์สเปียร์แมน

ผลการวิจัยพบว่า

พยาบาลส่วนใหญ่มีการปฏิบัติการดูแลทารกเกิดก่อนกำหนดแบบแกงการู ดังนี้ 1) ในระยะเตรียมความพร้อม พบว่าพยาบาลมีการเตรียมมารดา มีการประเมินความพร้อมของทารกมีการเตรียมสถานที่และอุปกรณ์ และมีการเตรียม พยาบาล 2) ในระยะก่อนการปฏิบัติแกงการู พบว่าพยาบาลมีการดูแลมารดา และมีการดูแลทารก 3) ในระยะ ปฏิบัติแกงการู พบว่าพยาบาลมีการดูแลมารดา และมีการดูแลทารก และ 4) ในระยะหลังการปฏิบัติแกงการู พบว่าพยาบาลมีการดูแลมารดา มีการดูแลทารก มีการจัดสถานที่และอุปกรณ์ และมีการบันทึกผลของพยาบาล

สำหรับปัจจัยที่สัมพันธ์กับการปฏิบัติการดูแลทารกเกิดก่อนกำหนดแบบแกงการูของพยาบาลอย่างมีนัยสำคัญ ทางสถิติ คือ ปัจจัยด้านองค์กร และปัจจัยด้านพยาบาล

ผลการวิจัยครั้งนี้ทำให้ได้ข้อเท็จจริงเกี่ยวกับการปฏิบัติการดูแลทารกเกิดก่อนกำหนดแบบ แกงการูของ พยาบาลและปัจจัยที่เกี่ยวข้อง ซึ่งสามารถนำไปเป็นแนวทางในการวางนโยบาย เพื่อส่งเสริมการปฏิบัติการดูแล ทารกเกิดก่อนกำหนดแบบแกงการูต่อไป

Abstract

Kangaroo care has shown to have many benefits to both parents and preterm infants. Nurses play an important role in forming the practices of kangaroo care for preterm infants. The purpose of this survey descriptive correlation research was to study Kangaroo Care Practiced for Preterm Infants by Nurses Including Relative Factors. The population of this study were 161 professional nurses, who were practicing at the neonatal intensive care unit and at the high risk neonatal unit at the 7 tertiary hospitals, northern Thailand. Data were collected from December 2009 to June 2010. The research instruments included 1) the Practices of Kangaroo Care for Preterm Infants among Nurses Questionnaires, 2) the Knowledge of Nurses about Kangaroo Care for Preterm Infants Questionnaires, and 3) the Related Factors to Nursing Practices Questionnaire. Data were analyzed using descriptive statistics, Point Biserial Correlation, and Spearman Rank Correlation.

The results of study

Revealed that most of the nurses reported having the following practices of kangaroo care for preterm infants. 1) In the preparation stage, nurses prepared mother, assessed the readiness of preterm infants, prepared the place and equipment, and prepared themselves. 2) In the pre-intervention stage, nurses provided care to mothers, and provided care to preterm infants. 3) In the intervention stage, nurses provided care to mothers, and provided care to preterm infants. 4) In the post-intervention stage, nurses provided care to mothers, provided care to preterm infants, arranged the place and equipment, and recorded their practices.

Factors that were statistically significantly associated with the practices of nurses in kangaroo care for preterm infants included organization factors and nurse factors.

The present report provides a fact-finding study of the Kangaroo Care Practiced for Preterm Infants by Nurses Including Relative Factors. Therefore, the information can be used for development of policy to promote practices of kangaroo care for preterm infants.
