Abstract

This action research aimed to develop and evaluate a clinical nursing practice guideline for assessing patient readiness for extubation after general anesthesia. The study was conducted at the Department of Anesthesiology, Khon Kaen Hospital, from November 2025 to June 2026, using Kemmis and McTaggart’s action research framework in two cycles. Cycle 1 involved situation analysis, guideline development, pilot implementation, and reflection for guideline refinement, while Cycle 2 involved implementation and evaluation of the revised guideline. Participants included nurse anesthetists and ASA Physical Status III patients undergoing non-emergency surgery under general anesthesia. Patient outcomes were compared between the pre- and post-implementation groups, with 95 patients in each group. Data were analyzed using descriptive statistics, the chi-square test, Fisher’s exact test, and the independent t-test. The results showed that readiness assessment for extubation varied among practitioners and lacked standardized criteria. The developed guideline consisted of two phases and four main steps. After implementation, no reintubation within 24 hours was observed, compared with 5.3% before implementation; the difference was not statistically significant (p=0.059). Upper airway obstruction decreased from 8.4% to 0% (p=0.007), while bradypnea decreased from 27.4% to 0% and desaturation decreased from 12.6% to 0% (p<0.001). Nurse anesthetists achieved 100% adherence to the guideline and reported the highest level of overall satisfaction (Mean=4.50, SD=0.12). The developed guideline was feasible for assessing patient readiness for extubation in the study setting.
