บทบาทพยาบาลในการเตรียมความพร้อมให้ผู้ดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันก่อนจำหน่ายออกจากโรงพยาบาล = Nurse’s role in preparing hospital discharge readiness for caregiver of the patient with acute myocardial infarction

Abstract

        การเตรียมความพร้อมให้กับผู้ดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันก่อนจำหน่ายออกจากโรงพยาบาลเป็นหัวใจสำคัญในการจำหน่ายผู้ป่วยออกจากโรงพยาบาล เพื่อดำรงไว้ซึ่งภาวะสุขภาพและคุณภาพชีวิตที่ดี อย่างไรก็ตามการดูแลตนเองที่บ้านของผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลัน เป็นเรื่องที่ยุ่งยากและซับซ้อน ต้องอาศัยทั้งความร่วมมือของสหวิชาชีพและการสนับสนุนจากผู้ดูแล การเตรียมความพร้อมก่อนจำหน่ายออกจากโรงพยาบาลที่สำคัญ สำหรับผู้ดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลัน ได้แก่ (1) การประเมินความพร้อมของผู้ป่วย (2) การประเมินความพร้อมของผู้ดูแลในการทำหน้าที่ตามบทบาท (3) การวางแผนร่วมกันระหว่าง พยาบาล ผู้ดูแล และ ผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลัน โดยการให้ข้อมูลเกี่ยวกับภาวะสุขภาพ และการฝึกทักษะในการดูแลผู้ป่วย ทั้งในเรื่องการรับรู้และการจัดการอาการผิดปกติที่เกิดขึ้น การควบคุมปัจจัยเสี่ยงของการเกิดโรคกล้ามเนื้อหัวใจตายซ้ำ การปรับเปลี่ยนพฤติกรรมการบริโภคอาหาร การดูแลเรื่องการทำกิจกรรม การออกกำลังกาย และการพักผ่อน (4) การเตรียมผู้ดูแลในการจัดการดูแลผู้ป่วยด้านจิตสังคม และ (5) การประสานงานกับทีมสุขภาพที่เกี่ยวข้อง ซึ่งหากผู้ดูแลสามารถดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันได้อย่างถูกต้องเหมาะสม ย่อมส่งผลให้อัตรากลับเข้ามารักษาซ้ำในโรงพยาบาลของผู้ป่วยลดน้อยลง ดังนั้นบทบาทของพยาบาลในการเตรียมความพร้อมของผู้ดูแล คือ การส่งเสริมความสามารถในการดูแลผู้ป่วย การให้ความรู้และคำแนะนำ การเป็นที่ปรึกษาและให้กำลังใจ ยังรวมถึงการเป็นผู้ประสานความร่วมมือระหว่างบุคลากรทีมสุขภาพ เพื่อให้ผู้ดูแลสามารถดูแลผู้ป่วยหลังจากจำหน่ายออกจากโรงพยาบาลมีประสิทธิภาพยิ่งขึ้นต่อไป

Nurse’s role in preparing hospital discharge readiness for caregiver of the patient with acute myocardial infarction

        Preparing hospital discharge readiness for caregivers of the patients with acute myocardial infarction is important for sustaining patient’s well being and quality of life. However, self-care of patients when staying at home is difficult and complex. It needs multidisciplinary collaboration, support from caregivers. In this regards, the significant caregivers’ readiness for hospital discharge are as follows: (1) assessing patients’ readiness, (2) assessing caregivers’ readiness in performing their roles, (3) planning together among nurses, caregivers and patients. These include giving information about health condition, practicing skills for caring of patients both perception and management of symptoms occurring to patient, controlling risk factors induced recurrent myocardial infarction, changing eating behavior, concerning patients’ activity, exercise and rest, (4) preparing caregivers in nurturing psychosocial care, and (5) coordinating multidisciplinary collaboration. The correct and appropriate care of patients that aforementioned helps decreasing readmission rate. Therefore, nurses’ important role in preparing hospital discharge readiness for caregivers of the patients with acute myocardial infarction is to promote caregivers’ ability in caring for patients that apart from giving knowledge, suggestion, counseling and providing morale support when facing with problems. It also includes coordinating multidisciplinary collaboration in order to enhance more efficiency care for patients after hospital discharge.
