Participatory community-based in promoting self-care of patients with chronic diseases to prevent Stroke

Community participation in self-care of chronically ill patients was encouraged patients to recognize their own health and to perceive the benefits to them, families, and communities. All sectors of the community were recognized the conditions, needs, and potential of the individual. Self-care of chronic patients to prevent Stroke is necessary and needs for cooperation from all sectors are as follows: (1) patients’ self-care and behavior modification of food consumption, medication, exercise, relaxation, stress reduction and participation in activities, (2) family-care with family support. (3) neighborhood with friend-care, (4) health volunteers with community-care team, participating in primary health screening, refer to health personnel advice and care with a list of health personnel contacted via SMART PHONE4.0, (5) healthcare team provide health education and empower through online doctor. (6) local public agency and non government agency support people’s health in all aspects. If all groups in community can perform their roles, patients can maintain their health effectively, reduce or prevent stroke and improve their quality of life.
