Nursing care in preterm infants with Patent Ductus Arteriosus

Abstract

In preterm, failure or delay in spontaneous closure of Ductus Arteriosus (DA), resulting in the condition of Patent Ductus Arteriosus (PDA), blood from the Aorta flow to the Pulmonary artery to the lung. A prolonged situation of PDA can be associated with several short- and long-term complications in preterm such as pulmonary edema, pulmonary hemorrhage, and chronic lung disease. Prophylaxis and treatment are necessary for PDA closure. The first-line therapy is an oral medical treatment with nonsteroidal anti-inflammatory drugs (NSAIDs), such as indomethacin, ibuprofen, and paracetamol with low to high side effects such as abdominal distension, necrotizing enterocolitis (NEC), and acute renal failure. The second-line therapy is operation including clipping PDA and PDA ligation in case of failure of medical treatment. Nurses should play important roles to care for preterm.  Nurses can earlier detect signs and symptoms of PDA, provide an accurate dosage of medicine, monitor post-operation care, prevent complications for efficacy, and higher safety for PDA closure in preterm.
