บทบาทพยาบาลและการสนับสนุนการตัดสินใจเลือกการรักษาในระยะสุดท้ายในผู้ป่วยวิกฤต : กรณีศึกษา

Abstract

การตัดสินใจเลือกแนวทางการรักษาสำหรับผู้ป่วยวิกฤตระยะสุดท้ายในหอผู้ป่วยวิกฤต (ไอ ซี ยู) มีความสำคัญเพื่อให้เกิดแนวทางการดูแลผู้ป่วยที่ชัดเจนและเหมาะสม โดยการเปลี่ยนเป้าหมายการดูแลจากการรักษาเพื่อการฟื้นหายเป็นการดูแลแบบประคับประคอง แม้ว่าการสนับสนุนให้ผู้ป่วยตัดสินใจเลือกการรักษาด้วยตนเองเป็นสิ่งที่ดี แต่ในความเป็นจริง ผู้ป่วยในภาวะวิกฤตอาจไม่มีความพร้อมในการตัดสินใจด้วยตนเองเนื่องจากปัญหาเรื่องการสื่อสารที่ถูกจำกัด ครอบครัวและญาติจึงเป็นบุคคลสำคัญที่ทำหน้าที่ตัดสินใจแทนผู้ป่วย การสื่อสารระหว่างแพทย์หรือพยาบาลกับครอบครัวและญาติและการสื่อสารในรูปแบบของการประชุมครอบครัวสามารถช่วยครอบครัวและญาติให้ได้รับข้อมูลประกอบการตัดสินใจ แพทย์มีบทบาทที่ชัดเจนในสนับสนุนการตัดสินใจระยะสุดท้ายโดยการให้ข้อมูลเกี่ยวกับการดำเนินโรคและการรักษา พยาบาลมีบทบาทเป็นศูนย์กลางการแลกเปลี่ยนข้อมูล ผู้อำนวยความสะดวกให้กับครอบครัวและญาติ เป็นตัวแทนปกป้องสิทธิผู้ป่วยเพื่อให้เกิดผลลัพธ์ที่ดีขึ้น และเป็นที่ปรึกษาที่ดีแก่ครอบครัวและญาติ ทำให้เกิดการตัดสินใจเลือกแนวทางการรักษาระยะสุดท้ายที่เหมาะสมตรงตามความต้องการของผู้ป่วย

Nursing roles and support in end-of-life decision making for critically ill patients : a case study

In the intensive care unit, providing treatment options for patients whose conditions have reached the end of life phase is essential as it turns the treatment goal from curative care to palliative care. Although supporting the patients’ treatment decisions seems worthwhile in terms of enhancing patient dignity, the fact appears to be that patients are unable to select preferred options by themselves due to limitations of communication. Consequently, surrogate decisions are always made by family members.  Effective communication between healthcare providers and family members in terms of family conferences will help families to make informed decisions with adequate information  on treatment options for end-stage patients. In order to respond based on patients’ needs, physicians have a clear role in definite decision-making by providing information about the disease and treatment progress. Nurses play a vital role in information exchanging, facilitators and patient advocating for good quality outcomes. In addition, nurses also act as good consultants to promote family to have appropriate decision making and responding to patients’ needs of end of life care.
