การพัฒนาแนวทางการส่งเสริมความสามารถในการใช้กระบวนการพยาบาลของนักศึกษาพยาบาล

Abstract

        การวิจัยนี้เป็นการวิจัยเชิงพรรณนามีวัตถุประสงค์เพื่อพัฒนาแนวทางการส่งเสริมความสามารถในการใช้กระบวนการพยาบาลของนักศึกษาพยาบาล กลุ่มตัวอย่างเป็นนักศึกษาพยาบาลชั้นปีที่ 2 จำนวน 84 คน คณะพยาบาลศาสตร์ มหาวิทยาลัยสวนดุสิต ที่เรียนวิชาปฏิบัติการพยาบาลพื้นฐาน ปีการศึกษา 2557 และอาจารย์พยาบาล 11 คน เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย1) แบบประเมินการวางแผนการพยาบาล 2) แบบประเมินความสามารถในการใช้กระบวนการพยาบาล 3)แบบสะท้อนคิดของนักศึกษาพยาบาลในการนำกระบวนการพยาบาลไปใช้ และ 4)แนวคำถามการสนทนากลุ่มนักศึกษาพยาบาล เก็บข้อมูลโดยการสนทนากลุ่มในนักศึกษาพยาบาล และอาจารย์ วิเคราะห์ข้อมูล โดยใช้สถิติเชิงพรรณนา และการวิเคราะห์เชิงเนื้อหา

        ผลการวิจัยพบว่า ความสามารถในการใช้กระบวนการพยาบาลของนักศึกษาพยาบาลในวิชาปฏิบัติการพยาบาลพื้นฐานอยู่ในระดับดี (mean = 4.11, S.D. = .291) ปัญหาการใช้กระบวนการพยาบาลของนักศึกษาพยาบาลพบมากที่สุดคือ ขาดทักษะในการประเมินภาวะสุขภาพ ส่วนกิจกรรมการจัดการเรียนการสอนที่ส่งเสริมความสามารถในการใช้กระบวนการพยาบาล คือ 1)การตั้งเป้าหมายในการเรียนของนักศึกษา 2) ฝึกการวางแผนการพยาบาลนำไปใช้ปฏิบัติกับผู้ป่วย 3) มีอาจารย์ให้คำแนะนำ 4) มีการประชุมปรึกษาก่อนและหลังการปฏิบัติงาน 5) มีการเรียนรู้กระบวนการพยาบาลแบบองค์รวม 6) พัฒนาทักษะการคิดวิเคราะห์อย่างเป็นระบบ 7) มีทัศนคติที่ดีต่อตนเอง และต่อบุคลากรทางสุขภาพอื่นๆ ส่วนแนวทางการส่งเสริมความสามารถในการใช้กระบวนการพยาบาลของนักศึกษาพยาบาลมี 3 องค์ประกอบ ได้แก่ 1) เตรียมความรู้ ฝึกทักษะการใช้กระบวนการพยาบาลในนักศึกษา 2) อาจารย์ควรปรับวิธีการสอน การมอบหมายงาน ประเมินผลการใช้กระบวนการพยาบาลในการปฏิบัติงาน 3) สถาบันหรือคณะพยาบาลควรมีการจัดการเรียนการสอนที่เอื้อต่อการใช้กระบวนการพยาบาลโดยเพิ่มชั่วโมงการฝึกการเขียนวางแผนการพยาบาล

        ผลการวิจัยครั้งนี้ สามารถนำไปใช้เป็นแนวทางการเรียนรู้ในการพัฒนาศักยภาพการใช้กระบวนการพยาบาลของนักศึกษาพยาบาลในวิชาการพยาบาลสาขาอื่นๆ ในสถาบันการศึกษาพยาบาล 

Development of a guideline to enhance nursing students’ abilities in  utilizing nursing process

        The aims of this descriptive research were to develop guidelines for enhancing the abilities to utilize nursing process among nursing students. The sample of this study (N=84) were second year nursing students of Faculty of Nursing, Suan Dusit University who were practiced in the “Fundamental Nursing Practicum” subject during the first semester, academic year 2014 and 11 nursing instructors. The research measurements were: 1) a nursing care plan assessment tool, 2) a nursing process utilizing capacity assessment tool, 3) a nursing students’ reflective sheet, and 4) a focus group interview. Quantitative data was analyzed by using the descriptive statistics, while the content analysis explored the qualitative results.

       The findings revealed that the levels of ability in using nursing process among the nursing students were good (mean=4.11, S.D. = .291). However, lack of skill of health assessment was reported as top issue in using the nursing process. The facilitating factors for motivating the students using the nursing process were reported in this study as: 1) setting the achievement goals by the students, 2) caring the patients following the nursing process, 3) guiding of care by nursing instructors, 4) participating the pre and post conference, 5) applying the holistic care in practice, 6) developing the critical thinking, and 7) promoting good attitude to oneself and other health care professionals. There were three key components that significantly promoted their abilities in using the nursing process: 1) the students should prepare their knowledge and are trained the nursing process skills, 2) the instructors should develop the appropriate teaching-learning methods, and 3) the institute or faculty of nursing should adjust the curriculum increasing the learning skills specifically targeted to apply the nursing process skills in practice.

        The results of this study should be applied as the learning guidelines to enhance the nursing students’ abilities in using the nursing process in other nursing subjects at school of nursing.
