การบำบัดทางการพยาบาลสำหรับครอบครัวผู้ป่วยวิกฤตที่เข้ารับการรักษาในตึกผู้ป่วยหนัก : การทบทวนวรรณกรรมอย่างเป็นระบบ

บทคัดย่อ

ภาวะเจ็บป่วยวิกฤตมีความรุนแรงและเกิดฃื้นเฉียบพลันก่อให้เกิดการสูญเสียหน้าที่ของอวัยวะที่สำคัญ หลายระบบและมีโอกาสเสียชีวิตสูง การดูแลรักษาในหอผู้ป่วยหนัก ภาวะความเจ็บป่วยที่คุกคามชีวิตและ ปัจจัยต่างๆ ที่ส่งผลกระทบทางด้านร่างกาย จิตใจและสังคมของครอบครัว เช่น ข้อจำกัดในของการเยี่ยม ไม่สามารถสื่อสารและตอบสนองความต้องการของผู้ป่วยได้ รวมทั้งความคลุมเครือของผลการรักษาและการ พยากรณ์โรค นอกจากนั้นการรับรู้ของครอบครัวต่อหอผู้ป่วยหนักว่าเป็นสถานที่เฉพาะสำหรับให้การดูแล ผู้ป่วยที่มีอาการอยู่ระหว่างความเป็นความตายทำให้คาดการณ์ผลทางลบในการรักษา ในบริบทของสังคม ไทยครอบครัวเป็นผู้ที่มีความหมายและเป็นแหล่งสนับสนุนที่ดีที่สุดเนื่องจากสามารถสื่อสารความรักและเป็น กำลังใจที่ดีแก่ผู้ป่วย การศึกษามีวัตถุประสงค์เพื่อวิเคราะห์องค์ความรูในการพัฒนาแนวปฏิบัติการพยาบาล ให้แก่สมาชิกครอบครัวผู้ป่วยวิกฤตที่เข้ารับการรักษาในตึกผู้ป่วยหนัก โดยสืบค้นจากแหล่งข้อมูลในช่วง ปี 1999-2009 เป็นงานวิจัยเชิงทดลองและกึ่งทดลองจำนวน 11 เรื่อง จัดอยู่ในระดับ 3.1 B จำนวน 9 เรื่อง ระดับ 3.2 B จำนวน 2 เรื่อง

ผลการศึกษา สามารถจำแนกผลลัพธ์ของการบำบัดการพยาบาลเป็น ความวิตกกังวล 6 เรื่อง (ร้อยละ 54.54) ความวิตกกังวลของสมาชิกกครอบครัวผู้ป่วยร่วมกับความพึงพอใจของพยาบาล 1 เรื่อง (ร้อยละ 9.01) ด้านความวิตกกังวลร่วมกับความพึงพอใจของสมาซิกครอบครัวผู้ป่วย 1 เรื่อง อีก 2 เรื่อง (ร้อยละ 18.18) เป็นผลด้านการปรับตัวและ 1 เรื่อง (ร้อยละ 9.01) เป็นผลลัพธ์ด้านความผาสุก และ พบรูปแบบการดูแลสมาซิกในครอบครัว 3 รูปแบบ ได้แก่ การให้ข้อมูลอย่างมีแบบแผน การช่วยเหลือและ สนับสนุนด้านอารมณ์และการให้สมาชิกครอบครัวมีส่วนร่วมในการดูแล ผลการศึกษาครั้งนี้ทำให้ได้องค์ ความรู้ที่สำคัญที่จะนำไปสู่การพัฒนาแนวปฏิบัติการพยาบาลบนพื้นฐานของมิตรภาพบำบัดเพื่อให้ผู้ป่วยและครอบครัวได้ รับบริการสุขภาพที่มีคุณภาพ สร้างความพึงพอใจ และความผาสุกของผู้ป่วย ครอบครัว และ ผู้ให้บริการ

Abstract

Critical ill condition is acute, severe, and affects multiple organs threatening the patientsû life. The nature of life threatening conditions together with the nature ICU admission affect familiesû physical and psycho-social such as the limitations of visiting duration, lack communication and response to the love one, inadequate information regarding the patient's treatment and prognosis. In addition, the family's perception of ICU admission is between live and death leading to negative expectation and treatment outcome. In Thai context, family members are very important supports for the patients. Family members who understand the patients well are the best resource of mental support because they can communicate love and empowering to the patients. This literature review aimed to investigate the available scientific knowledge of nursing therapeutic guideline for families of critically ill in ICU. The data were collected and analyzed experimental or quasi-experimental research studies related to reducing families, stressful of critically ill patients in intensive care unit. A search was carried out for the period of 1999-2009. There were a total of 11 research studies consisting of 9 studies in level 3.1 B and 2 studies in 3.2B.

In this study, we report finding on research outcomes mostly consisted of decrease anxiety (54.54%), anxiety and nurses' satisfaction (9.01%), anxiety and families member's satisfaction (18.18%), coping (9.01%), and families member's well being (9.10%). In addition, the strategies of providing families of critically ill patients in intensive care units are divided into three categories: provision of information, emotional support and participation of care. Results of this approach are able to be applied to guide quality of nursing practice based on humanized care to enhance patients, families and health providers' satisfaction and well being.
